Symptoms:
Symptoms of Polycystic Ovary Syndrome (PCOS) often begin around the time a person has their first menstrual period. In some cases, symptoms may appear later after someone has been having menstrual cycles for several years.
The symptoms of PCOS can vary from person to person. Doctors usually diagnose PCOS when at least two of the following signs are present:
Irregular menstrual periods
Irregular or infrequent periods are common in people with PCOS. Some may have fewer menstrual cycles in a year, while others may experience long gaps between periods. Menstrual bleeding may also last longer than usual. For example, a person may have fewer than nine periods in a year or cycles that occur more than 35 days apart. These irregularities may also make it more difficult to become pregnant.
High levels of androgen
PCOS can cause increased levels of androgen, a hormone associated with male characteristics. High androgen levels may lead to excessive hair growth on the face or body, a condition known as hirsutism. In some cases, severe acne and hair thinning or hair loss similar to male-pattern baldness may also occur.
Polycystic ovaries
In PCOS, the ovaries may become enlarged and develop many small follicles around their outer edges. These follicles contain immature eggs and may prevent the ovaries from functioning normally.
Symptoms of PCOS are often more noticeable or severe in individuals who have obesity.
When to See a Doctor
You should consult a healthcare professional if you are concerned about irregular menstrual cycles, have difficulty becoming pregnant, or notice signs of increased androgen levels. These signs may include new or excessive hair growth on the face or body, persistent acne, or thinning hair similar to male-pattern baldness.
Causes:
The exact cause of PCOS is not fully understood. However, several factors may contribute to the development of this condition:
Insulin resistance
Insulin is a hormone produced by the pancreas that helps the body’s cells use sugar for energy. When cells become resistant to insulin, blood sugar levels can rise. In response, the body may produce more insulin to try to control blood sugar levels.
High insulin levels may stimulate the body to produce excess androgen. This can interfere with ovulation, the process where the ovaries release eggs.
A common sign of insulin resistance is the appearance of dark, velvety patches of skin, often found on the back or sides of the neck, under the arms, in the groin area, or under the breasts. Increased appetite and weight gain may also occur.
Low-grade inflammation
White blood cells release substances in response to infection or injury, which causes inflammation. Research suggests that people with PCOS may have a form of long-term, low-grade inflammation. This condition may stimulate the ovaries to produce higher levels of androgens, which can increase the risk of heart and blood vessel problems.
Heredity
Genetic factors may play a role in PCOS. Studies indicate that certain genes may be associated with the condition. Having a family history of PCOS may increase the likelihood of developing it.
Excess androgen production
In PCOS, the ovaries may produce higher-than-normal levels of androgen. Too much androgen can disrupt ovulation, meaning eggs may not mature or be released regularly from the follicles. High androgen levels can also lead to symptoms such as excessive hair growth and acne.

Diagnosis:
There is no single test that can confirm Polycystic Ovary Syndrome (PCOS). Doctors usually diagnose the condition by reviewing symptoms, medical history, and performing several tests. A healthcare provider may first ask about menstrual patterns, weight changes, medications you take, and any other medical conditions. A physical examination may also be done to look for signs such as excessive hair growth, acne, or indications of insulin resistance.
Your healthcare provider may recommend the following tests:
Pelvic examination
During a pelvic exam, the doctor checks the reproductive organs for any abnormalities such as masses, growths, or structural changes.
Blood tests
Blood tests are used to measure hormone levels. These tests can help rule out other conditions that may cause symptoms similar to PCOS, such as irregular periods or high androgen levels. Additional blood tests may include fasting cholesterol and triglyceride levels. A glucose tolerance test may also be performed to evaluate how the body processes sugar.
Ultrasound
An ultrasound helps examine the ovaries and the thickness of the uterine lining. A small device called a transducer is usually placed in the vagina. It produces sound waves that create images of the ovaries and uterus on a computer screen.
If PCOS is diagnosed, additional tests may be recommended to check for possible complications. These may include:
-
Regular monitoring of blood pressure, blood sugar levels, cholesterol, and triglycerides
-
Screening for depression and anxiety
-
Screening for obstructive sleep apnea
Treatment:
Treatment for PCOS focuses on managing the symptoms or health concerns a person is experiencing. These may include infertility, excessive hair growth, acne, or weight-related problems. Treatment plans may involve lifestyle changes, medications, or both.
Lifestyle Changes
Doctors often recommend lifestyle adjustments as the first step in managing PCOS. Weight loss through a balanced, low-calorie diet combined with regular moderate exercise can help improve symptoms. Even a small weight reduction, such as losing about 5% of body weight, may make a significant difference.
Weight loss can also increase the effectiveness of medications and may improve fertility. A healthcare provider or registered dietitian can help create a personalized plan for healthy weight management.
Medications
To help regulate menstrual cycles, doctors may recommend:
Combination birth control pills
These pills contain estrogen and progestin. They help reduce androgen production and regulate hormone levels. This can lower the risk of endometrial cancer and improve symptoms such as irregular bleeding, excessive hair growth, and acne.
Progestin therapy
Taking progestin for about 10 to 14 days every one to two months can help regulate menstrual cycles and protect the lining of the uterus from cancer. However, this treatment does not reduce androgen levels and does not prevent pregnancy. Progestin-only pills or progestin-releasing intrauterine devices may be considered if pregnancy prevention is also desired.
Medications to Support Ovulation
If pregnancy is desired, a doctor may recommend medications that help stimulate ovulation:
Clomiphene
An oral medication that helps stimulate ovulation and is usually taken early in the menstrual cycle.
Letrozole (Femara)
Originally used to treat breast cancer, this medication can also help stimulate the ovaries to release eggs.
Metformin
This medication is commonly used to treat type 2 diabetes. It can improve insulin resistance and lower insulin levels. In some cases, it may help restore ovulation and support weight management.
Gonadotropins
These hormone medications are given through injections and can stimulate the ovaries to release eggs.
In some cases, assisted reproductive techniques such as in vitro fertilization (IVF) may be considered if other treatments do not help with pregnancy.
Treatments for Excess Hair Growth and Acne
To manage symptoms related to high androgen levels, doctors may recommend:
Birth control pills
These help reduce androgen production and may improve excessive hair growth and acne.
Spironolactone (Aldactone)
This medication blocks the effects of androgen on the skin and may reduce excessive hair growth and acne. It should not be used during pregnancy and requires reliable birth control during treatment.
Eflornithine (Vaniqa)
A topical cream that may slow down facial hair growth.
Hair removal methods
Procedures such as electrolysis and laser hair removal can help remove unwanted hair. Electrolysis uses a small needle and electrical current to destroy hair follicles. Laser hair removal uses concentrated light to reduce hair growth. Multiple sessions are usually required. Temporary methods like shaving, plucking, or hair-removal creams can also be used but may need to be repeated regularly.
Acne treatments
Acne can be treated with various medications, including oral medicines and topical creams or gels. A healthcare provider can recommend suitable treatment options.

(Femara)
(Aldactone)
(Vaniqa)
