Polycystic Kidney Disease

Polycystic kidney disease (পলিসিস্টিক কিডনি রোগ) is a disorder in which numerous fluid-filled cysts develop in the body, mainly in the kidneys. As these cysts grow over time, they can cause the kidneys to enlarge and gradually lose their ability to function properly. In most cases, PKD is inherited, meaning it is passed down from parents to their children.

Cysts are round, sac-like structures that contain fluid. They are noncancerous. In people with PKD, these cysts can vary in size and number. Some cysts may become very large, and when many cysts are present or when they grow significantly, they can damage kidney tissue.

In addition to the kidneys, cysts may also develop in other organs such as the liver, pancreas, and occasionally other parts of the body. The condition can lead to several health complications, including high blood pressure and kidney failure.

The severity of polycystic kidney disease can differ widely from person to person. While the disease cannot always be prevented, certain treatments and healthy lifestyle habits may help reduce kidney damage and lower the risk of complications.

পলিসিস্টিক কিডনি রোগ এমন একটি অবস্থা যেখানে শরীরে অনেকগুলো তরলভর্তি সিস্ট তৈরি হয়, প্রধানত কিডনিতে। সময়ের সাথে সাথে এই সিস্টগুলো বড় হতে থাকে এবং কিডনির আকার বৃদ্ধি পায়। এর ফলে ধীরে ধীরে কিডনির স্বাভাবিক কাজ করার ক্ষমতা কমে যেতে পারে। অধিকাংশ ক্ষেত্রে এই রোগটি বংশগত, অর্থাৎ এটি পরিবার থেকে উত্তরাধিকারসূত্রে পাওয়া যায়।

সিস্ট হলো গোলাকার থলের মতো গঠন যার ভেতরে তরল থাকে। এগুলো ক্যান্সার নয়। পলিসিস্টিক কিডনি রোগে সিস্টগুলোর আকার ও সংখ্যা ভিন্ন হতে পারে। অনেক সময় সিস্ট খুব বড় হয়ে যেতে পারে। যদি অনেক সিস্ট থাকে বা সেগুলো বড় হয়ে যায়, তাহলে কিডনির টিস্যু ক্ষতিগ্রস্ত হতে পারে।

এই রোগে কিডনির পাশাপাশি লিভার, অগ্ন্যাশয় এবং শরীরের অন্য কিছু অঙ্গেও সিস্ট তৈরি হতে পারে। এই অবস্থার কারণে উচ্চ রক্তচাপ এবং কিডনি বিকল হওয়ার মতো গুরুতর জটিলতা দেখা দিতে পারে।

পলিসিস্টিক কিডনি রোগের তীব্রতা ব্যক্তি ভেদে অনেক ভিন্ন হতে পারে। যদিও এই রোগ পুরোপুরি প্রতিরোধ করা সবসময় সম্ভব নয়, তবে কিছু চিকিৎসা পদ্ধতি এবং স্বাস্থ্যকর জীবনযাপন কিডনির ক্ষতি কমাতে এবং জটিলতার ঝুঁকি হ্রাস করতে সাহায্য করতে পারে।

Symptoms:

The symptoms of polycystic kidney disease can vary from person to person. Some common symptoms include:

  • High blood pressure

  • Pain in the abdomen, side, or back

  • Blood in the urine

  • A feeling of fullness or heaviness in the abdomen

  • Enlargement of the abdomen due to enlarged kidneys

  • Headaches

  • Kidney stones

  • Kidney failure

  • Urinary tract infections or kidney infections

Many people may have polycystic kidney disease for several years without realizing it because symptoms may not appear in the early stages.

If you notice symptoms that may be related to polycystic kidney disease, it is important to consult a healthcare professional. People who have a close family member, such as a parent, sibling, or child with this condition, should also speak with a healthcare provider about screening and early detection.

Causes:

Polycystic kidney disease is caused by changes in certain genes. In most cases, the condition runs in families and is passed from parents to children. However, in some cases, a gene change can occur spontaneously in a child even when neither parent carries the gene.

There are two main types of polycystic kidney disease, and each type is caused by different gene changes.

Autosomal Dominant Polycystic Kidney Disease (ADPKD)
This is the most common inherited form of the disease. Symptoms usually begin between the ages of 30 and 40.

Only one parent needs to have this condition for it to be passed to their children. If one parent has ADPKD, each child has about a 50 percent chance of inheriting the disease.

Autosomal Recessive Polycystic Kidney Disease (ARPKD)
This form of the disease is much less common. Symptoms often appear shortly after birth, although in some cases they may not develop until later in childhood or during the teenage years.

For a child to develop ARPKD, both parents must carry the changed gene. If both parents carry the gene, each child has a 25 percent chance of developing the condition.

পলিসিস্টিক কিডনি ডিজিজের লক্ষণ ব্যক্তি ভেদে ভিন্ন হতে পারে। সাধারণ লক্ষণগুলোর মধ্যে রয়েছে:

  • উচ্চ রক্তচাপ

  • পেট, পাশ বা পিঠে ব্যথা

  • প্রস্রাবে রক্ত দেখা যাওয়া

অনেক মানুষ বহু বছর ধরে পলিসিস্টিক কিডনি ডিজিজে আক্রান্ত থাকতে পারেন কিন্তু তা বুঝতে পারেন না, কারণ শুরুতে লক্ষণ স্পষ্ট নাও হতে পারে।

যদি আপনার মধ্যে এই রোগের কিছু লক্ষণ দেখা যায়, তাহলে একজন স্বাস্থ্যসেবা প্রদানকারীর সাথে পরামর্শ করা উচিত। এছাড়া যদি আপনার বাবা-মা, ভাইবোন বা সন্তানের মধ্যে কারও এই রোগ থাকে, তাহলে স্ক্রিনিং বা পরীক্ষা সম্পর্কে ডাক্তারের সাথে আলোচনা করা গুরুত্বপূর্ণ।

পলিসিস্টিক কিডনি ডিজিজের কারণ

পলিসিস্টিক কিডনি ডিজিজ নির্দিষ্ট জিনের পরিবর্তনের কারণে হয়। বেশিরভাগ ক্ষেত্রে এই রোগটি বংশগত, অর্থাৎ এটি বাবা-মা থেকে সন্তানের মধ্যে চলে আসে। তবে কিছু ক্ষেত্রে শিশুর শরীরে নিজ থেকেই জিনের পরিবর্তন হতে পারে, যদিও বাবা-মায়ের কারও সেই পরিবর্তিত জিন না-ও থাকতে পারে।

পলিসিস্টিক কিডনি ডিজিজের দুটি প্রধান ধরন রয়েছে, এবং এগুলো ভিন্ন ধরনের জিন পরিবর্তনের কারণে হয়।

অটোসোমাল ডমিন্যান্ট পলিসিস্টিক কিডনি ডিজিজ (ADPKD)
এটি এই রোগের সবচেয়ে সাধারণ বংশগত ধরন। সাধারণত ৩০ থেকে ৪০ বছর বয়সের মধ্যে এর লক্ষণ দেখা দিতে শুরু করে।

এই ক্ষেত্রে বাবা-মায়ের একজনের এই রোগ থাকলেই সন্তানের মধ্যে এটি যাওয়ার সম্ভাবনা থাকে। যদি একজন অভিভাবকের ADPKD থাকে, তাহলে প্রতিটি সন্তানের এই রোগ হওয়ার সম্ভাবনা প্রায় ৫০ শতাংশ।

অটোসোমাল রিসেসিভ পলিসিস্টিক কিডনি ডিজিজ (ARPKD)
এই ধরনটি তুলনামূলকভাবে অনেক কম দেখা যায়। এর লক্ষণ সাধারণত জন্মের পরপরই দেখা দিতে পারে। তবে কিছু ক্ষেত্রে শৈশবের পরে বা কিশোর বয়সে লক্ষণ প্রকাশ পেতে পারে।

এই ধরনের রোগ হওয়ার জন্য বাবা-মা দুজনেরই পরিবর্তিত জিন থাকা প্রয়োজন। যদি বাবা-মা দুজনই এই জিন বহন করেন, তাহলে প্রতিটি সন্তানের এই রোগ হওয়ার সম্ভাবনা প্রায় ২৫ শতাংশ।
