Symptoms:
The symptoms of narcolepsy often become more noticeable during the first few years after onset and then usually continue throughout life. Common symptoms include the following:
Extreme daytime sleepiness
Excessive daytime sleepiness is usually the earliest and most prominent symptom. People with narcolepsy feel very sleepy during the day, making it difficult to concentrate, stay alert, and perform daily activities. They may fall asleep suddenly without warning, even while working, talking, or eating. Sleep episodes can happen anywhere and may last from a few minutes to about half an hour. After waking up, the person may feel refreshed for a short time but soon becomes sleepy again. Falling asleep while driving can be especially dangerous.
Automatic behaviors
Some individuals may continue performing activities during brief sleep episodes without realizing it. For example, they may keep writing, typing, or even driving while partially asleep. After waking, they often do not remember what they did, and the task is usually done poorly.
Sudden loss of muscle tone (Cataplexy)
Cataplexy involves a sudden weakness or loss of muscle control that can last from a few seconds to several minutes. It is often triggered by strong emotions, particularly positive ones such as laughter or excitement. However, fear, anger, or surprise can also cause it. A person’s head may suddenly drop, speech may become slurred, or the knees may buckle, leading to a fall. Some people experience cataplexy rarely, while others may have multiple episodes each day. Not everyone with narcolepsy has cataplexy.
Sleep paralysis
People with narcolepsy may experience temporary inability to move or speak while falling asleep or waking up. These episodes usually last only a few seconds or minutes but can be frightening. The person is aware of what is happening and can remember the episode afterward. Sleep paralysis can also occur in people without narcolepsy.
Hallucinations
Vivid and sometimes frightening hallucinations may occur when falling asleep or waking up. When they happen while falling asleep, they are called hypnagogic hallucinations, and when they occur upon waking, they are called hypnopompic hallucinations. For example, a person may believe they see someone in the room who is not actually there. These experiences feel very real because the person is not fully asleep.
Changes in REM sleep
Rapid eye movement (REM) sleep is the stage when most dreaming occurs. Normally, REM sleep begins about 60 to 90 minutes after falling asleep. In narcolepsy, REM sleep may begin within 15 minutes and can occur at unusual times, including during the day.
Other sleep-related problems
People with narcolepsy may also have other sleep disorders, such as obstructive sleep apnea, where breathing repeatedly stops and starts during sleep. Some may physically act out their dreams, known as REM sleep behavior disorder. Others may struggle with falling asleep or staying asleep, which is called insomnia.

A healthcare professional should be consulted if daytime sleepiness interferes with work, education, relationships, or overall quality of life.
Causes:
The exact cause of narcolepsy is not fully understood. In type 1 narcolepsy, levels of a brain chemical called hypocretin, also known as orexin, are very low. This chemical helps regulate wakefulness and REM sleep.
Low hypocretin levels are strongly associated with cataplexy. Experts believe that the loss of hypocretin-producing cells may be caused by an autoimmune reaction, in which the body’s immune system mistakenly attacks its own cells.
Genetics may also play a role, but the chance of passing narcolepsy from a parent to a child is very low, estimated at about 1 to 2 percent.
In some cases, narcolepsy has been linked to exposure to the H1N1 flu virus and to a specific H1N1 vaccine that was used in parts of Europe.
Typical Sleep Pattern vs Narcolepsy
Normally, sleep begins with non-rapid eye movement (NREM) sleep, during which brain activity gradually slows. After about an hour, the brain enters REM sleep, when most dreaming occurs.
In narcolepsy, this process is disrupted. A person may enter REM sleep very quickly, sometimes almost immediately after falling asleep. REM-related features such as muscle weakness, paralysis, and dream-like hallucinations may occur while the person is awake or only partially asleep.

Sleep history
A detailed review of sleep habits and symptoms is an important first step. Patients are often asked to complete the Epworth Sleepiness Scale, a questionnaire that measures how likely a person is to fall asleep during everyday situations, such as sitting quietly after a meal.
Sleep diary and activity tracking
Patients may be asked to keep a record of their sleep and wake times for one to two weeks. This helps compare sleep patterns with levels of daytime alertness. In some cases, a wrist-worn device called an actigraph is used to monitor movement, rest periods, and sleep timing.
Overnight sleep study (Polysomnography)
This test is done in a medical facility and records body functions during sleep. Small electrodes placed on the scalp measure brain activity, while other sensors track heart rate, breathing, eye movement, and leg movement. This test helps identify abnormal sleep patterns and rule out other sleep disorders.
Multiple sleep latency test
This daytime test measures how quickly a person falls asleep. The patient is asked to take four or five scheduled naps, each spaced two hours apart. People with narcolepsy tend to fall asleep quickly and enter REM sleep much sooner than normal.
Genetic testing and lumbar puncture
In selected cases, genetic testing may be done to assess the risk of type 1 narcolepsy. If needed, a lumbar puncture may be recommended to measure hypocretin levels in the spinal fluid. These tests are usually performed only at specialized centers.

Medicines
Several types of medications are used to treat narcolepsy:
Stimulants

Other newer medications, such as solriamfetol and pitolisant, are also used to improve wakefulness. Pitolisant may additionally help reduce cataplexy.
Some patients may require stronger stimulants like methylphenidate or amphetamines. These drugs are effective but may cause side effects such as nervousness, increased heart rate, and dependence.
Antidepressants
Certain antidepressants help suppress REM sleep and are used to control cataplexy, hallucinations, and sleep paralysis. These include serotonin and norepinephrine reuptake inhibitors and selective serotonin reuptake inhibitors. Possible side effects include weight gain, difficulty sleeping, and digestive issues.
Older tricyclic antidepressants may also be effective for cataplexy, but they are used less often due to side effects such as dry mouth and dizziness.
Oxybate medications
Sodium oxybate and other oxybate-based medicines are highly effective in treating cataplexy and improving nighttime sleep quality. They may also help reduce daytime sleepiness. Newer formulations contain lower amounts of sodium.
These medications can cause side effects such as nausea, bed-wetting, and sleepwalking. They must not be combined with alcohol, opioid pain medicines, or other sleep medications because of the risk of serious breathing problems.
Patients should always inform their healthcare professional about other medicines they are taking, as some drugs may interact with narcolepsy treatments.
Over-the-counter medications, especially allergy and cold remedies, can cause drowsiness and may worsen symptoms. People with narcolepsy are often advised to avoid them.
Future treatments
