Male Infertility

Approximately one in seven couples faces infertility, meaning they have been unable to conceive despite having regular, unprotected sexual intercourse for a year or longer. Male infertility is a factor in up to half of these cases.

Male infertility can result from low sperm production, problems with sperm function, or blockages that prevent sperm from being delivered. Various causes such as medical conditions, injuries, chronic illnesses, and lifestyle factors may contribute to male infertility.

Although difficulty conceiving can be stressful and emotionally challenging, there are several treatment options available to help address male infertility.

Symptoms:

The primary sign of male infertility is difficulty in conceiving a child. Often, there may be no other visible signs.

In some cases, underlying conditions such as hormonal imbalances, inherited disorders, dilated veins around the testicles, or blockages that prevent sperm delivery may cause noticeable symptoms. Common signs include:

  • Difficulty with sexual function, such as problems with ejaculation, reduced sexual desire, or difficulty maintaining an erection

  • Pain, swelling, or a lump in the testicle area

  • Recurrent respiratory infections

  • Loss of sense of smell

  • Abnormal breast growth (gynecomastia)

  • Reduced facial or body hair or other signs of chromosomal or hormonal abnormalities

  • Low sperm count (fewer than 15 million sperm per milliliter of semen or a total sperm count of less than 39 million per ejaculation)

When to See a Doctor

Consult a doctor if you have been unable to conceive after a year of regular, unprotected intercourse, or sooner if you experience:

  • Erectile or ejaculation problems, low libido, or other sexual difficulties

  • Pain, swelling, or a lump in the testicles

  • History of testicular, prostate, or sexual health issues

  • Previous groin, penis, scrotum, or testicle surgery

  • If your partner is over 35 years old

Causes:

Male fertility is a complex process that requires:

  1. Healthy sperm production: Proper development of reproductive organs, adequate hormone levels (including testosterone), and functioning testicles.

  2. Sperm transport: Sperm must travel through delicate tubes to mix with semen and be ejaculated.

  3. Sufficient sperm count: Low sperm count reduces the chance of fertilization.

  4. Functional sperm: Sperm must be able to move and fertilize the egg effectively.

Medical Causes:

  • Varicocele: Swollen veins in the testicles, often reversible, affecting sperm quantity and quality.

  • Infections: Infections of the testicles or epididymis, or sexually transmitted infections, may reduce sperm production or cause blockages.

  • Ejaculation problems: Retrograde ejaculation occurs when semen flows into the bladder instead of out of the penis, caused by conditions such as diabetes, medications, or surgery.

  • Anti-sperm antibodies: Immune system attacks sperm mistakenly.

  • Tumors: Cancers or benign tumors, or their treatments, can impair fertility.

  • Undescended testicles: Testicles that fail to descend properly during fetal development can reduce fertility.

  • Hormonal imbalances: Disorders affecting testosterone or other reproductive hormones.

  • Tubule defects: Blockages in sperm-carrying tubes from injury, surgery, infection, or inherited conditions.

  • Chromosome abnormalities: Genetic disorders like Klinefelter’s syndrome, cystic fibrosis, or Kallmann’s syndrome.

  • Sexual problems: Erectile dysfunction, premature ejaculation, painful intercourse, or anatomical abnormalities.

  • Celiac disease: Gluten sensitivity may contribute to infertility.

  • Medications and surgeries: Certain drugs or surgical procedures can impair sperm production.

Environmental Causes:

  • Exposure to chemicals, pesticides, heavy metals, or radiation

  • Overheating the testicles through frequent use of hot tubs, saunas, tight clothing, or prolonged sitting

  • Long-term laptop use may slightly raise scrotal temperature

Lifestyle and Other Causes:

  • Drug use: Anabolic steroids, cocaine, or marijuana can reduce sperm production

  • Alcohol consumption can lower testosterone and impair sperm production

  • Smoking reduces sperm count and quality

  • Obesity affects sperm quality and hormone balance

পুরুষ অপ্রস্রুতির প্রধান লক্ষণ হলো সন্তানধারণে অক্ষমতা। অনেক সময় অন্য কোনো প্রকাশিত লক্ষণ দেখা যায় না।

এক বছর নিয়মিত এবং সুরক্ষাহীন যৌনক্রিয়ার পরেও সন্তানধারণে ব্যর্থ হলে বা নিম্নলিখিত সমস্যার ক্ষেত্রে:

  • ইরেকশন বা শুক্রাণু নির্গমনের সমস্যা, কম যৌন ইচ্ছা

  • অন্ডকোষে ব্যথা, ফোলা বা গাদা

পুরুষ অপ্রস্রুতির কারণ

পুরুষের প্রজনন প্রক্রিয়া জটিল এবং এর জন্য প্রয়োজন:

  1. সুস্থ শুক্রাণু উৎপাদন: প্রজনন অঙ্গের সঠিক বৃদ্ধি, যথাযথ হরমোন (টেস্টোস্টেরনসহ) এবং অন্ডকোষের কার্যকারিতা।

  2. শুক্রাণু পরিবহন: শুক্রাণু সঠিকভাবে নালী দিয়ে সেমিনের সাথে মিশে নির্গত হতে হবে।

  3. যথেষ্ট শুক্রাণু সংখ্যা: কম শুক্রাণু সংখ্যা নিষেকের সম্ভাবনা হ্রাস করে।

  4. শুক্রাণুর কার্যকারিতা: শুক্রাণু সক্রিয় এবং яйানু নিষেক করতে সক্ষম হতে হবে।

চিকিৎসাজনিত কারণ:

  • ভেরিকোসেল: অন্ডকোষের রক্তনালীর ফোলা, যা শুক্রাণুর মান ও সংখ্যা কমায়।

  • সংক্রমণ: অন্ডকোষ বা এপিডিডিমিসের সংক্রমণ বা যৌনসংক্রামক রোগ শুক্রাণুর উৎপাদন কমাতে পারে।

  • শুক্রাণু নির্গমনের সমস্যা: রেট্রোগ্রেড ইজাকুলেশন বিভিন্ন রোগ, ওষুধ বা সার্জারির কারণে হতে পারে।

  • অ্যান্টি-শুক্রাণু অ্যান্টিবডি: ইমিউন সিস্টেম ভুলক্রমে শুক্রাণুকে আক্রমণ করে।

  • টিউমার: ক্যান্সার বা অ-ক্যান্সার টিউমার এবং তাদের চিকিৎসা প্রজনন ক্ষমতা হ্রাস করতে পারে।

জীবনধারা ও অন্যান্য কারণ:

  • ড্রাগ ব্যবহার: অ্যানাবোলিক স্টেরয়েড, কোকেইন বা মারিজুয়ানা শুক্রাণু কমাতে পারে

  • মদ্যপান: টেস্টোস্টেরন কমায় এবং শুক্রাণু উৎপাদন কমায়

  • ধূমপান: শুক্রাণু সংখ্যা ও মান কমায়

  • অতিরিক্ত ওজন: শুক্রাণুর মান ও হরমোনে প্রভাব ফেলে
