Low Sperm Count

A low sperm count (লো স্পার্ম কাউন্ট)refers to having fewer sperm in the semen than is considered normal. Semen is the fluid released from the penis during ejaculation.

Medically, a low sperm count is known as oligospermia, while a complete absence of sperm is called azoospermia. A sperm count is generally considered low if there are fewer than 15 million sperm per milliliter of semen.

Having a low sperm count can reduce the chances of one of the sperm fertilizing an egg, making conception more difficult. There are treatments available that can help improve the likelihood of pregnancy for couples facing this condition.

Symptoms:

The most noticeable symptom of a low sperm count is difficulty in achieving pregnancy. Many men may not experience any other obvious symptoms.

Sometimes, an underlying health condition can cause a low sperm count along with additional symptoms such as:

  • Sexual problems: Low libido or difficulty achieving or maintaining an erection (erectile dysfunction).

  • Pain, swelling, or a lump in the testicles.

  • Reduced facial or body hair, or other signs of hormonal or genetic disorders.

See a healthcare professional if you and your partner have been unable to conceive after a year of regular, unprotected sexual activity. Seek medical advice sooner if you experience:

  • Erectile or ejaculation problems, low sex drive, or other sexual difficulties.

  • Pain, swelling, or a lump in the testicle area.

  • History of testicular, prostate, or sexual issues.

  • Previous surgeries on the groin, testicles, penis, or scrotum.

Causes:

Sperm production depends on healthy testicles and hormones produced by certain brain glands. After sperm are made in the testicles, they travel through small tubes and mix with semen, which is released from the penis during ejaculation. Any problem in this system can reduce sperm count.

Irregular sperm shape or movement can also lower fertility. In many cases, the exact cause remains unknown.

Medical Causes:

  • Varicocele: Swelling of veins in the testicles, a common cause of male infertility. Surgery may improve sperm count, movement, and shape.

  • Infections: Some infections, including sexually transmitted infections like gonorrhea or HIV, or infections causing testicular swelling, can affect sperm.

  • Blocked tubes: Injuries, infections, surgeries, or genetic conditions like cystic fibrosis can block sperm pathways.

  • Genetic conditions: Syndromes like Klinefelter, Kallmann, or Kartagener can reduce sperm production.

Environmental & Lifestyle Factors:

  • Exposure to industrial chemicals, pesticides, heavy metals, radiation, or excessive heat.

  • Use of anabolic steroids, cocaine, marijuana, alcohol, or tobacco.

  • Obesity, stress, or emotional problems.

Other Causes:

  • Certain medications, surgeries, or fertility testing issues may temporarily affect sperm count.

কারণসমূহ

শুক্রাণু তৈরির জন্য অণ্ডকোষ এবং মস্তিষ্কের নির্দিষ্ট হরমোন গ্রন্থি সুস্থভাবে কাজ করতে হয়। শুক্রাণু তৈরি হওয়ার পর ছোট টিউব দিয়ে সিমেনে মিশে পিনিস থেকে নির্গত হয়। এই প্রক্রিয়ায় কোনো সমস্যা থাকলে শুক্রাণুর সংখ্যা কমে যেতে পারে।

শুক্রাণুর গঠন বা গতিবিধি অস্বাভাবিক থাকলেও ফার্টিলিটি কমতে পারে। অনেক সময় এর সঠিক কারণ জানা যায় না।

চিকিৎসাজনিত কারণ:

  • ভ্যারিসোকেল: অণ্ডকোষের শিরার ফোলা। শল্যচিকিৎসায় শুক্রাণুর সংখ্যা ও গুণমান উন্নত হতে পারে।

  • সংক্রমণ: যৌনমাধ্যমে সংক্রমণ বা অণ্ডকোষে ফোলা সৃষ্টি করে এমন সংক্রমণ শুক্রাণুকে প্রভাবিত করতে পারে।

  • স্খলন সমস্যা: রেট্রোগ্রেড স্খলন তখন হয় যখন সিমেন মূত্রথলিতে চলে যায়। কারণ হতে পারে ডায়াবেটিস, মেরুদণ্ডের আঘাত, কিছু অস্ত্রোপচার বা ওষুধ।

  • টিউমার: ক্যান্সার বা অ-ক্যান্সার টিউমার শুক্রাণু উৎপাদনকে প্রভাবিত করতে পারে।

  • অণ্ডকোষ না নামা: জন্মের সময় অণ্ডকোষ না নামলে পরবর্তীতে ফার্টিলিটি কমে যায়।
