Symptoms:
Common signs that someone has lice include:
-
Itching: Persistent itching on the scalp, body, or pubic area.
-
Visible lice: Adult lice may be seen on the scalp, body, clothing, or pubic hair. They are usually about the size of a sesame seed or slightly larger.
-
Lice eggs (nits): These tiny eggs attach firmly to hair shafts. They are most noticeable near the ears and along the hairline at the neck and can resemble dandruff, but they are not easily brushed out.
-
Sores or bumps: Scratching can cause small red bumps on lighter skin and slightly darker marks on darker skin. These may become infected if left untreated.
-
Bite marks: Small areas of blood or crust may appear, often around the waist, groin, upper thighs, and pubic region.
-
Swollen lymph nodes: Some children with head lice may have enlarged lymph nodes in the neck. Lymph nodes are part of the body’s immune system.
When to See a Doctor
Most cases of lice can be treated without professional care. However, you should contact a healthcare provider if you are unsure about what you see or for pubic lice, to rule out other infections. Children with pubic lice should also be evaluated for signs of possible sexual abuse.
Causes:
Lice are insects that feed on human blood and live on the head, body, or pubic area. Infection occurs when you come into direct contact with lice or their eggs.
-
Head lice: Female head lice attach eggs to hair shafts with a sticky substance. Eggs hatch in 6–9 days.
-
Body lice: Usually lay eggs in clothing seams.
-
Pubic lice: Most commonly spread through sexual contact. In children, pubic lice may indicate sexual abuse.
Transmission of Lice
Lice crawl but cannot jump or fly. They can survive 1–2 days off the human body. Common ways lice spread include:
-
Direct contact: Head-to-head or body-to-body contact, often between family members or children at school or play.
-
Shared clothing or bedding: Clothes stored together, pillows, blankets, or towels can carry lice.
-
Shared personal items: Combs, brushes, headphones, and stuffed toys can spread lice.
-
Furniture contact: Sitting or lying on cloth-covered furniture recently used by someone with lice.

Diagnosis:
A healthcare provider or you can detect lice using a magnifying glass and a fine-toothed comb.
-
Head lice: Diagnosis is made by spotting live lice in the hair or on the scalp. Healthcare providers also look for eggs, called nits, using a special nit comb. Finding nits alone does not confirm active lice.
-
Body lice: Diagnosis is made by observing crawling lice or eggs on clothing seams. Body lice may be seen on clothing, bedding, or temporarily on the skin while feeding.
-
Pubic lice: Diagnosis is made by finding moving lice or nits in the pubic area or other coarse hair, such as chest hair, eyebrows, or eyelashes.
Treatment:
There are several ways to eliminate lice and their eggs:
-
Head lice: Wet combing, smothering, or dehydrating techniques can remove lice. Over-the-counter or prescription medications can also be used.
-
Topical treatments: Products containing permethrin or pyrethrin are commonly applied to affected areas. Permethrin is safe for children 2 months and older; pyrethrin is suitable for those 2 years and older. Follow package instructions carefully, then remove nits with the included comb. Repeat combing every 2–3 days for 2–3 weeks. A second application is usually needed on day 9 or 10.
-
Body lice: Begin with thorough bathing using soap and water, and wash clothes and bedding in hot water. If medication is needed, apply it to affected areas at bedtime, avoiding the face, and rinse in the morning. Repeat on day 9 if necessary.
When Treatments Fail
In some regions, over-the-counter lice treatments may not be effective. Incorrect use can also reduce effectiveness. If standard treatments fail for head lice, your healthcare provider may suggest prescription options:
-
Spinosad: Suitable for children 6 months and older. Apply to dry hair, leave for 10 minutes, rinse with warm water, and avoid shampooing for 24–48 hours. Repeat after 7 days if lice persist.
-
Malathion: Suitable for children 6 years and older. Apply to dry hair for 8–12 hours, then rinse carefully. Avoid shampooing for 24–48 hours. This product is flammable, so keep away from heat sources. Repeat after 7 days if necessary.
-
Ivermectin: Oral medicine for head or body lice if other treatments fail. Suitable for children over 15 kg (33 lbs). Repeat after 7–10 days if needed. Not recommended during pregnancy or breastfeeding. A lotion form (Sklice) is also available; follow professional guidance before repeating.
