Left Ventricular Hypertrophy

Left ventricular hypertrophy (লেফট ভেন্ট্রিকুলার হাইপারট্রফি) is the thickening of the walls of the heart’s lower left chamber, known as the left ventricle. The left ventricle is the main pumping chamber of the heart.

When left ventricular hypertrophy occurs, the thickened heart wall can become stiff, causing an increase in blood pressure within the heart. These changes make it more difficult for the heart to pump blood efficiently. Over time, the heart may struggle to pump with enough force to meet the body’s needs.

The most common cause of LVH is uncontrolled high blood pressure. Possible complications include irregular heart rhythms, known as arrhythmias, and heart failure.

Treatment for left ventricular hypertrophy depends on its underlying cause. Management may include medications or, in some cases, surgical procedures.

Symptoms:

Left ventricular hypertrophy (LVH) usually develops gradually. Some people may not notice any symptoms, especially in the early stages.

LVH itself may not directly cause symptoms, but as the heart experiences more strain, symptoms can appear, including:

  • Shortness of breath, especially when lying down

  • Swelling of the legs

  • Chest pain, often during physical activity

  • Rapid, fluttering, or pounding heartbeats (palpitations)

  • Fainting or feeling lightheaded

Seek emergency medical care if:

  • Chest pain lasts more than a few minutes

  • Severe difficulty breathing occurs

  • Severe lightheadedness or loss of consciousness happens

  • Sudden severe headache, trouble speaking, or weakness on one side of the body occurs

For mild symptoms like occasional shortness of breath or palpitations, consult a healthcare professional.

People with high blood pressure or other risk factors for LVH should have regular heart checkups.

Causes:

LVH develops when the heart’s lower left chamber, the left ventricle, faces increased stress. Over time, this strain causes the heart wall to thicken and sometimes the chamber itself to enlarge.

Causes of LVH include:

  1. High blood pressure (hypertension): The most common cause. Long-term high blood pressure makes the left ventricle work harder, causing it to grow thicker. Controlling blood pressure can reduce LVH and sometimes reverse it.

  2. Narrowing of the aortic valve (aortic stenosis): The valve between the left ventricle and the aorta narrows, making the heart work harder to pump blood.

  3. Intense athletic training: Long-term strength and endurance training can enlarge the heart muscle. This is sometimes called “athlete’s heart.” The long-term effects on heart stiffness and disease are still unclear.

  4. Genetic conditions: Some inherited conditions can thicken the heart muscle, such as:

    • Hypertrophic cardiomyopathy: Genetic changes cause the heart muscle to thicken, making pumping harder. It can occur even without high blood pressure. A child of an affected parent has a 50% chance of inheriting the gene.

    • Amyloidosis: Protein buildup around the heart and other organs interferes with normal function. Familial forms can affect nerves and kidneys.

বাম ভেন্ট্রিকুলার হাইপারট্রফি (LVH) সাধারণত ধীরে ধীরে বৃদ্ধি পায়। অনেকের প্রাথমিক পর্যায়ে কোনও উপসর্গ দেখা যায় না।

LVH নিজে সরাসরি উপসর্গ সৃষ্টি নাও করতে পারে, তবে হৃদয়ের উপর চাপ বাড়ার সাথে সাথে নিচের উপসর্গগুলো দেখা দিতে পারে:

  • শুয়ে থাকার সময় শ্বাসকষ্ট

  • পায়ের ফোলা

  • শারীরিক কসরতের সময় বুকে ব্যথা

  • দ্রুত, ধাক্কা বা ঝাপসা হৃদস্পন্দন (পালপিটেশন)

  • মাথা ঘোরা বা হঠাৎ হতাশা

জরুরি চিকিৎসা প্রয়োজন হলে:

  • বুকের ব্যথা কয়েক মিনিটের বেশি স্থায়ী হলে

  • তীব্র শ্বাসকষ্ট হলে

  • হঠাৎ মাথা ঘোরা বা অজ্ঞান হওয়া

  • হঠাৎ তীব্র মাথাব্যথা, কথা বলার অসুবিধা, বা শরীরের এক পাশে দুর্বলতা

হালকা উপসর্গ যেমন মাঝে মাঝে শ্বাসকষ্ট বা পালপিটেশন হলে, একজন চিকিৎসকের পরামর্শ নিন।

যাদের উচ্চ রক্তচাপ বা LVH-এর ঝুঁকি বেশি, তাদের নিয়মিত হৃদয় পরীক্ষা করা উচিত।

কারণ

বাম ভেন্ট্রিকুলার হাইপারট্রফি তখন ঘটে যখন হৃদয়ের নিচের বাম চেম্বার, অর্থাৎ বাম ভেন্ট্রিকল, অতিরিক্ত চাপের সম্মুখীন হয়। সময়ের সাথে সাথে এই চাপ হৃদয় প্রাচীরকে পুরু করে এবং কখনও কখনও চেম্বারকেও বড় করে।

LVH-এর প্রধান কারণগুলো:

  1. উচ্চ রক্তচাপ (হাইপারটেনশন): সবচেয়ে সাধারণ কারণ। দীর্ঘমেয়াদী উচ্চ রক্তচাপ বাম ভেন্ট্রিকুলকে বেশি কাজ করতে বাধ্য করে, যার ফলে এটি পুরু হয়ে যায়। রক্তচাপ নিয়ন্ত্রণ করলে LVH হ্রাস পেতে পারে এবং কখনও কখনও বিপরীত প্রভাবও দেখা দিতে পারে।

  2. অ্যোর্টিক ভালভের সংকীর্ণতা (অ্যোর্টিক স্টেনোসিস): বাম ভেন্ট্রিকুল এবং অ্যাোর্টার মধ্যে থাকা ভালভ সংকীর্ণ হলে, হৃদয়কে রক্ত পাম্প করতে বেশি পরিশ্রম করতে হয়।

  3. তীব্র ক্রীড়া প্রশিক্ষণ: দীর্ঘমেয়াদী শক্তি ও ধৈর্য্য কসরত হৃদয়কে বড় করতে পারে। এটি কখনও কখনও “অ্যাথলেটের হৃদয়” বলা হয়। দীর্ঘমেয়াদে এর প্রভাব এখনও সম্পূর্ণভাবে জানা যায়নি।

  4. জিনগত সমস্যা: কিছু বংশগত অবস্থার কারণে হৃদয় পুরু হয়ে যায়, যেমন:

    • হাইপারট্রফিক কার্ডিওমায়োপ্যাথি: জিনগত পরিবর্তনের কারণে হৃদয় পুরু হয় এবং রক্ত পাম্প করা কঠিন হয়। এটি উচ্চ রক্তচাপ ছাড়াও ঘটতে পারে। প্রভাবিত পিতামাতার সন্তানের ৫০% সম্ভাবনা এই জিন পাওয়ার।

    • অ্যামিলয়েডোসিস: হৃদয় ও অন্যান্য অঙ্গের চারপাশে প্রোটিন জমা হয়, যা অঙ্গের কার্যকারিতা ব্যাহত করে। বংশগত হলে এটি নার্ভ ও কিডনিতেও প্রভাব ফেলতে পারে।
