Symptoms:
The signs of hemophilia depend on how much clotting factor is missing. People with mild hemophilia may only bleed excessively after surgery or serious injuries. Those with severe hemophilia can bleed easily, sometimes for no apparent reason.
Common signs of spontaneous bleeding include:
-
Unexplained or heavy bleeding from cuts, injuries, surgery, or dental work
-
Multiple large or deep bruises
-
Unusual bleeding after vaccinations
-
Pain, swelling, or stiffness in joints
-
Blood in urine or stool
-
Nosebleeds without a clear cause
-
In infants, unusual irritability
Bleeding into the brain
In severe cases, even a minor bump on the head can cause bleeding in the brain. Although rare, this is a serious complication. Symptoms may include:
-
Persistent, painful headache
-
Repeated vomiting
-
Drowsiness or unusual sleepiness
-
Double vision
-
Sudden weakness or loss of coordination
-
Seizures
When to seek medical attention
Get emergency care if you or your child experiences:
-
Signs of bleeding in the brain
-
An injury that won’t stop bleeding
-
Swollen, painful joints that are warm to touch
Causes:
Normally, when you bleed, blood cells clump together to form a clot, stopping the bleeding. Clotting factors are proteins in the blood that help platelets form clots. Hemophilia occurs when one of these clotting factors is missing or present in low amounts.
Congenital hemophilia
Most hemophilia cases are inherited, meaning the person is born with the condition. It is classified based on which clotting factor is low:
Acquired hemophilia
Some individuals develop hemophilia without a family history. This occurs when the immune system attacks clotting factors 8 or 9. Possible triggers include:
Inheritance
Hemophilia is usually linked to a gene on the X chromosome. Females have two X chromosomes, while males have one X and one Y chromosome. This is why hemophilia is much more common in boys and is usually passed from mother to son. Most women carrying the gene do not show symptoms, but some may have mild bleeding if their clotting factor levels are slightly reduced.

Diagnosis:
Severe hemophilia is often identified during the first year of life, while mild cases may not become obvious until adulthood. Some people discover they have hemophilia after experiencing excessive bleeding during surgery or medical procedures.
Blood tests that measure clotting factors can confirm a deficiency and determine the severity of hemophilia.
For families with a history of hemophilia, genetic testing may be used to identify carriers, helping individuals make informed decisions about pregnancy. During pregnancy, it is also possible to test whether the unborn baby has hemophilia, although this testing carries certain risks. Discuss potential benefits and risks with your doctor.
Treatment:
The primary treatment for severe hemophilia is replacing the missing clotting factor through an intravenous infusion.
-
Replacement therapy can be given during a bleeding episode or on a regular schedule at home to prevent bleeding. Some individuals may receive continuous therapy.
-
Replacement clotting factor can be derived from donated blood or produced in a laboratory as recombinant clotting factors.
Other treatment options include:
-
Desmopressin: In some mild cases, this hormone can stimulate the body to release more clotting factor. It can be administered as a slow injection or a nasal spray.
-
Emicizumab (Hemlibra): A newer medication that helps prevent bleeding episodes in hemophilia A without using clotting factors.
-
Clot-preserving medications (anti-fibrinolytics): These help prevent clots from breaking down.
-
Fibrin sealants: Applied directly to wounds to aid clotting and healing, especially useful for dental procedures.
-
Physical therapy: Can relieve joint pain and stiffness caused by internal bleeding. Severe joint damage may require surgery.
-
First aid for minor cuts: Applying pressure and a bandage usually stops bleeding. Ice packs can reduce minor skin bleeding, and ice pops may help slow oral bleeding.
