Foot Drop

Foot drop(ফুট ড্রপ) also known as drop foot, refers to difficulty in lifting the front part of the foot. This condition can cause the toes to drag along the ground while walking, leading to an abnormal gait.

Foot drop itself is not a disease but rather a symptom of an underlying problem involving the muscles, nerves, or structural components of the leg or spine. It may result from conditions such as nerve injury, muscular disorders, or neurological diseases.

In some cases, foot drop may improve over time, especially if the cause is treatable. However, for others, it can become a long-term condition. Management often includes wearing an ankle-foot orthosis (AFO) — a type of brace that helps hold the foot in a proper position, making walking safer and more stable.

Early diagnosis and appropriate treatment are essential to manage the condition effectively and improve mobility.

কিছু ক্ষেত্রে ফুট ড্রপ সাময়িক হতে পারে এবং চিকিৎসার মাধ্যমে উন্নতি ঘটতে পারে। তবে অনেকের ক্ষেত্রে এটি দীর্ঘস্থায়ী হয়ে পড়ে। এ অবস্থায় অ্যাংকল-ফুট অর্থোসিস (AFO) নামক একটি বিশেষ ব্রেস ব্যবহার করা হয়, যা পা সঠিকভাবে স্থানে ধরে রাখতে সহায়তা করে এবং হাঁটার সময় ভারসাম্য বজায় রাখে।

প্রাথমিক পর্যায়ে সঠিক রোগ নির্ণয় এবং চিকিৎসা গ্রহণ করলে রোগীর চলাফেরা সহজ ও স্বাভাবিক করা সম্ভব।

Symptoms

Foot drop makes it hard to lift the front part of the foot, which can cause the toes to drag while walking. To avoid this, a person may lift their leg higher than normal, similar to climbing stairs. This walking pattern is known as steppage gait and can result in the foot slapping the ground with each step. In some people, there may also be numbness on the top of the foot or toes.

Foot drop can affect just one foot or, in some cases, both.

If you notice your toes dragging while walking, you should speak with a medical professional. Delaying treatment can lead to reduced mobility and even long-term pain, which may affect your ability to perform daily activities and lower your quality of life.

Causes

Foot drop occurs when the muscles responsible for lifting the front part of the foot become weak or paralyzed. This can be due to several underlying reasons:

  • Nerve Damage: The most frequent cause is damage to the peroneal nerve, which controls the muscles that lift the foot. This nerve may become compressed due to knee injuries or during surgeries like hip or knee replacement.

  • Spinal Nerve Compression: A pinched nerve in the lower spine can also affect foot movement. Individuals with diabetes are more likely to experience nerve problems, increasing their risk for foot drop.

  • Muscle or Nerve Disorders: Genetic conditions like muscular dystrophy, or neurological disorders such as Charcot-Marie-Tooth disease and polio, may also lead to foot drop.

  • Brain or Spinal Cord Disorders: Conditions like stroke, multiple sclerosis (MS), or amyotrophic lateral sclerosis (ALS) can disrupt nerve signals to the leg, resulting in foot drop.

লক্ষণসমূহ

ফুট ড্রপে আক্রান্ত ব্যক্তির জন্য পায়ের সামনের অংশ উঠানো কঠিন হয়ে পড়ে, ফলে হাঁটার সময় পায়ের আঙুল মাটিতে ঘষে যেতে পারে। এই সমস্যা এড়াতে অনেকেই হাঁটার সময় পা অস্বাভাবিকভাবে ওপরের দিকে তোলে, যেন সিঁড়ি ভাঙছে। এই হাঁটার ধরনকে স্টেপেজ গেইট বলা হয়, এবং এতে প্রতি পদক্ষেপে পা জোরে মাটিতে পড়ে যেতে পারে। কারও কারও ক্ষেত্রে পায়ের উপরিভাগ ও আঙুলে অসাড়ভাবও অনুভূত হতে পারে।

এই অবস্থা এক পায়ে হতে পারে, আবার কখনো দুই পায়েই হতে পারে।

কখন চিকিৎসকের পরামর্শ নেওয়া উচিত

যদি হাঁটার সময় আপনার পায়ের আঙুল মাটিতে লেগে যায় বা টেনে চলতে হয়, তাহলে অবশ্যই চিকিৎসকের পরামর্শ নিন। চিকিৎসা না করালে এর ফলে চলাফেরায় সমস্যা হতে পারে এবং দীর্ঘমেয়াদে ব্যথা সৃষ্টি হতে পারে, যা দৈনন্দিন জীবনে প্রভাব ফেলতে পারে।

কারণসমূহ

ফুট ড্রপ সাধারণত পায়ের সামনের অংশ উঠানোর জন্য দায়ী পেশিগুলোর দুর্বলতা বা পক্ষাঘাতের কারণে হয়। এর পেছনে থাকতে পারে বিভিন্ন কারণ:

  • স্নায়ুর ক্ষতি: ফুট ড্রপের সবচেয়ে সাধারণ কারণ হলো পেরোনিয়াল স্নায়ুর ক্ষতি, যা পায়ের সামনের অংশ তুলতে সহায়তা করে। হাঁটুর আঘাত বা হিপ ও হাঁটুর অস্ত্রোপচারে এই স্নায়ু চাপে পড়ে যেতে পারে।

  • স্পাইনাল স্নায়ু সংক্রমণ: কোমরের হাড়ে “পিঞ্চড নার্ভ” থাকলে সেখান থেকে পায়ে স্নায়ু সংকেত পৌঁছাতে ব্যাঘাত ঘটে, যা ফুট ড্রপের কারণ হতে পারে। ডায়াবেটিস রোগীদের ক্ষেত্রে স্নায়ুজনিত সমস্যা বেশি দেখা যায়, ফলে তাদের ঝুঁকি বেশি।

  • পেশি বা স্নায়ুর রোগ: জন্মগত রোগ যেমন মাসকুলার ডিস্ট্রোফি বা স্নায়ুবিক রোগ যেমন পোলিও এবং চারকট-মেরি-টুথ রোগেও ফুট ড্রপ হতে পারে।

  • মস্তিষ্ক ও স্পাইনাল কর্ডের রোগ: স্ট্রোক, মাল্টিপল স্ক্লেরোসিস (MS) বা অ্যামিওট্রফিক ল্যাটেরাল স্ক্লেরোসিস (ALS)-এর মতো রোগে মস্তিষ্ক থেকে পায়ে সঠিক স্নায়ু সংকেত পৌঁছাতে না পারলে ফুট ড্রপ দেখা দিতে পারে।
