Emphysema

Emphysema (এমফাইসেমা )is a chronic lung condition that makes it difficult to breathe. It occurs when the walls of the tiny air sacs in the lungs, called alveoli, become damaged. In healthy lungs, these air sacs expand when you inhale and contract when you exhale, helping to move air in and out efficiently. However, in emphysema, the alveoli lose their elasticity and become weakened or ruptured. This makes it harder for the lungs to release old air and take in fresh, oxygen-rich air.

Common symptoms include shortness of breath, especially during physical activity, and wheezing while exhaling. The severity of the condition can vary from person to person.

Smoking is the most common cause of emphysema. While there is no cure, treatments are available to manage symptoms and may slow the progression of the disease. Early diagnosis and lifestyle changes can help improve quality of life.

এমফাইসেমা একটি দীর্ঘমেয়াদি ফুসফুসের রোগ, যা শ্বাস নিতে কষ্ট হয়। এই রোগে ফুসফুসের ছোট ছোট বাতাসের থলি (অ্যালভিওলাই) ক্ষতিগ্রস্ত হয়। স্বাভাবিকভাবে, এই থলিগুলো শ্বাস নেওয়ার সময় প্রসারিত হয় এবং শ্বাস ছাড়ার সময় সংকুচিত হয়, যা বাতাস চলাচলে সাহায্য করে। কিন্তু এমফাইসেমায় এই থলিগুলো তাদের স্থিতিস্থাপকতা হারায় এবং অনেক সময় ফেটে যায়। ফলে ফুসফুস থেকে বাতাস বের হতে সমস্যা হয় এবং নতুন, অক্সিজেন-সমৃদ্ধ বাতাস প্রবেশ করতে পারে না।

সাধারণ লক্ষণগুলোর মধ্যে আছে হাঁটার বা কাজ করার সময় শ্বাসকষ্ট এবং শ্বাস ছাড়ার সময় শোঁ শোঁ শব্দ। রোগটির তীব্রতা ব্যক্তি ভেদে ভিন্ন হতে পারে।

ধূমপান হলো এমফাইসেমার প্রধান কারণ। এই রোগের কোনো স্থায়ী চিকিৎসা নেই, তবে উপসর্গ নিয়ন্ত্রণে রাখা যায় এবং রোগের গতি ধীর করা সম্ভব। প্রাথমিক পর্যায়ে শনাক্তকরণ ও জীবনযাত্রার পরিবর্তন জীবনমান উন্নত করতে সহায়ক হতে পারে।

Symptoms

Emphysema can develop slowly, and you might not notice symptoms for years. Early signs often appear gradually and may include:

  • Shortness of breath, especially during physical activity — the most common symptom

  • Wheezing or a whistling sound when exhaling

  • Frequent coughing

  • A feeling of tightness or pressure in the chest

  • Fatigue or feeling unusually tired

  • Weight loss and swelling in the ankles as the condition worsens

As breathing becomes more difficult, many people start avoiding activities that make them breathless. Over time, even resting can feel challenging due to reduced lung function.

Emphysema is one of the two main forms of chronic obstructive pulmonary disease (COPD). The other is chronic bronchitis, where the bronchial tubes become inflamed and produce excess mucus, narrowing the airways. Both conditions often occur together and are commonly referred to as COPD.

Exacerbations

At times, symptoms may suddenly become worse for several days or weeks — this is known as an acute exacerbation. Without timely medical care, it can lead to serious complications such as respiratory failure.

Triggers for exacerbations can include:

  • Respiratory infections

  • Air pollution

  • Exposure to irritants that cause lung inflammation

Seek medical help promptly if you experience:

  • Increased coughing

  • More mucus production

  • Greater difficulty in breathing

When to See a Doctor

You should consult a healthcare provider if you have had unexplained shortness of breath for several months, especially if it’s worsening or affecting your daily life. Don’t assume it’s due to age or lack of fitness.

Seek emergency care immediately if:

  • You’re struggling to breathe or speak

  • Your lips or nails turn a bluish or gray color during physical activity

  • Others notice confusion or lack of alertness

Causes

Emphysema is typically caused by long-term exposure to airborne irritants. Common causes include:

  • Cigarette smoking — the leading cause

  • Chemical fumes and industrial vapors

  • Dust exposure, especially in workplaces

In rare cases, emphysema is caused by an inherited condition called alpha-1-antitrypsin (AAT) deficiency. This condition lowers levels of a protective protein that helps prevent lung damage. People with this deficiency often have a family history of emphysema and develop symptoms at a younger age. It may also lead to liver problems.

লক্ষণসমূহ

এমফাইসেমা ধীরে ধীরে গড়ে ওঠে এবং অনেক সময় বছরের পর বছর ধরে কোনো লক্ষণ দেখা যায় না। উপসর্গগুলো ধীরে ধীরে শুরু হয় এবং এর মধ্যে রয়েছে:

  • হাঁটার বা পরিশ্রম করার সময় শ্বাসকষ্ট — এটি সবচেয়ে সাধারণ লক্ষণ

  • শ্বাস ছাড়ার সময় শোঁ শোঁ বা সুরযুক্ত শব্দ

  • ঘন ঘন কাশি

  • বুকে চাপ বা জড়তা অনুভব করা

  • অতিরিক্ত ক্লান্তিভাব বা দুর্বলতা

  • রোগের অগ্রগতির সাথে ওজন হ্রাস ও গোড়ালিতে ফোলাভাব

অনেক সময় শ্বাসকষ্ট এড়াতে মানুষ দৈনন্দিন কাজকর্ম এড়িয়ে চলতে শুরু করে। ফলে উপসর্গগুলো স্পষ্ট হয় না যতক্ষণ না তারা দৈনন্দিন জীবনে বাধা সৃষ্টি করে। এক সময় বিশ্রামের অবস্থায়ও শ্বাস নিতে সমস্যা হতে পারে।

এমফাইসেমা হলো ক্রনিক অবস্ট্রাকটিভ পালমোনারি ডিজিজ (COPD)-এর দুটি সাধারণ ধরন-এর একটি। অন্যটি হলো ক্রনিক ব্রঙ্কাইটিস, যেখানে শ্বাসনালী ফুলে যায় এবং অতিরিক্ত কফ তৈরি করে, যা বাতাস চলাচলে বাধা দেয়। এ দুটি রোগ একসঙ্গে হতে পারে এবং একত্রে COPD নামে পরিচিত।

বাড়াবাড়ি (Exacerbations)

চিকিৎসা চলমান থাকলেও মাঝে মাঝে উপসর্গগুলো হঠাৎ করে অনেকটা খারাপ হয়ে যেতে পারে — একে বলে একিউট এক্সাসারবেশন। সময়মতো চিকিৎসা না পেলে এটি ফুসফুসের কাজ বন্ধ করে দিতে পারে।

যেসব কারণে এক্সাসারবেশন হতে পারে:

  • শ্বাসতন্ত্রের সংক্রমণ

  • বায়ু দূষণ

  • ফুসফুসে প্রদাহ তৈরি করে এমন যেকোনো উপাদান

যদি আপনি লক্ষ্য করেন যে:

  • কাশি বেড়েছে

  • কফ বেশি হচ্ছে

  • শ্বাস নিতে অতিরিক্ত কষ্ট হচ্ছে

তবে দ্রুত চিকিৎসা নেয়া জরুরি।

কখন ডাক্তার দেখাবেন

যদি আপনি কয়েক মাস ধরে অজানা কারণে শ্বাসকষ্টে ভুগে থাকেন এবং এটি ধীরে ধীরে খারাপ হচ্ছে বা দৈনন্দিন কাজ বাধাগ্রস্ত করছে, তাহলে অবশ্যই চিকিৎসকের পরামর্শ নিন। বয়সের দোষ দিয়ে উপসর্গ উপেক্ষা করবেন না।

নিচের ক্ষেত্রে জরুরি বিভাগে যেতে হবে:

  • শ্বাস নিতে বা কথা বলতে কষ্ট হলে

  • শারীরিক কাজকর্মের সময় ঠোঁট বা নখ নীলচে বা ধূসর রঙ ধারণ করলে

  • অন্যরা বুঝতে পারেন যে আপনি সজাগ বা মনোযোগী নন

কারণসমূহ

এমফাইসেমা সাধারণত দীর্ঘমেয়াদি বায়ুবাহিত উত্তেজক পদার্থে সংস্পর্শের ফলে হয়। এর সাধারণ কারণগুলো হলো:

  • সিগারেট ধূমপান — প্রধান কারণ

  • রাসায়নিক ধোঁয়া ও গ্যাস, বিশেষত কর্মস্থলে

  • ধুলোবালি, বিশেষত পেশাগত ক্ষেত্রে

বিরলভাবে, একটি জেনেটিক সমস্যা (alpha-1-antitrypsin বা AAT ঘাটতি) থেকেও এমফাইসেমা হতে পারে। AAT হলো এমন একটি প্রোটিন যা ফুসফুসকে ক্ষতি থেকে রক্ষা করে। এই ঘাটতি থাকলে সাধারণত পরিবারে এমফাইসেমার ইতিহাস থাকে এবং কম বয়সেই উপসর্গ শুরু হয়। এতে লিভার সমস্যা হওয়ার সম্ভাবনাও থাকে।
