Symptoms
Symptoms of diphtheria usually start within 2 to 5 days after infection. Common signs and symptoms include:
-
A thick, grayish layer forming over the throat and tonsils
-
Sore throat and a hoarse voice
-
Swollen lymph nodes in the neck
-
Difficulty breathing or unusually fast breathing
-
Runny nose
-
Fever and chills
-
General weakness and tiredness
In some people, the infection may cause only mild symptoms, or no symptoms at all. These individuals are called carriers, as they can spread the bacteria to others without appearing sick themselves.
Skin (Cutaneous) Diphtheria
Apart from the throat, diphtheria can also affect the skin. This type usually causes:
-
Redness, swelling, and pain at the infected skin area
-
Ulcer-like sores, often covered with a grayish membrane
Cutaneous diphtheria is more common in tropical regions but can also occur elsewhere, especially among people living in overcrowded places with poor hygiene.
When to See a Doctor
Seek medical help immediately if you or your child has been in contact with someone diagnosed with diphtheria. If you are unsure whether your child is fully vaccinated, consult your doctor for advice and possible vaccination updates.
Causes
Diphtheria is caused by a bacterium called Corynebacterium diphtheriae. The bacteria usually grow on the surface of the throat or skin.
The main ways diphtheria spreads include:
-
Airborne Transmission: When an infected person coughs or sneezes, small droplets containing the bacteria can spread through the air and infect others nearby. This is especially a risk in crowded areas.
-
Contaminated Objects: Touching items like used tissues, towels, or utensils that carry the bacteria can also cause infection.
-
Contact with Infected Wounds: Touching an open sore or wound infected with diphtheria bacteria can spread the disease.
Even people who don’t show symptoms but carry the bacteria can unknowingly infect others, especially those who are unvaccinated.

(Cutaneous Diphtheria)
Diagnosis
Doctors usually suspect diphtheria when a sick child presents with a sore throat and a thick grayish layer covering the throat and tonsils. To confirm the diagnosis, a sample is taken from the throat membrane and tested in a laboratory for the presence of Corynebacterium diphtheriae.
For skin infections (cutaneous diphtheria), doctors may take a small tissue sample from the affected area and send it for laboratory analysis.
Because diphtheria can become severe very quickly, doctors typically start treatment as soon as they suspect the infection, even before lab test results are available.
Treatment
Diphtheria is a medical emergency that requires immediate and intensive care.
First, doctors check if the patient’s airway is open and clear. If there is any risk of blocked breathing, a tube may be inserted into the throat to assist breathing until the swelling reduces.
Main treatments include:
-
Antibiotics: Medicines like penicillin or erythromycin are given to kill the bacteria and help control the infection. Antibiotics also reduce the risk of spreading the disease to others.
-
Antitoxin: A special medication is used to neutralize the toxins produced by the diphtheria bacteria. This antitoxin is usually injected into a vein or muscle. Before giving this medication, doctors may conduct allergy tests to ensure the patient won’t have a severe reaction.
People receiving treatment for diphtheria are often admitted to a hospital, usually in an isolation ward or intensive care unit, to prevent the infection from spreading to others, especially to those who are not vaccinated.
Preventive Treatment for Close Contacts
If you have been exposed to someone diagnosed with diphtheria, it is important to seek medical advice quickly. The doctor may recommend:
-
Antibiotics to prevent the disease from developing
-
A booster dose of the diphtheria vaccine, especially if your last vaccination was given a long time ago
People who are found to be carriers (infected but not showing symptoms) also receive antibiotic treatment to eliminate the bacteria from their system and prevent further spread.
