Diabetic ketoacidosis (ডায়াবেটিক কিটোঅ্যাসিডোসিস )is a critical complication related to diabetes. This condition occurs when the body fails to produce enough insulin, a hormone essential for allowing sugar (glucose) to enter the body’s cells to provide energy.

When insulin levels are too low, the body starts breaking down fat for energy instead. This process leads to the release of acids called ketones, which accumulate in the bloodstream. Excessive ketone buildup makes the blood acidic, leading to diabetic ketoacidosis.

If left untreated, this condition can become life-threatening.

People with diabetes or those at risk of developing diabetes should be aware of the warning signs of diabetic ketoacidosis. Early recognition and prompt medical care are crucial to avoid serious health complications.

Symptoms

Symptoms of diabetic ketoacidosis can develop quickly, often within a single day. For some people, these symptoms may be the first indication that they have diabetes.

Common symptoms include:

  • Feeling very thirsty

  • Frequent urination

  • Nausea or vomiting

  • Abdominal pain

  • Feeling very weak or tired

  • Shortness of breath

  • Breath with a fruity or sweet smell

  • Confusion or difficulty thinking clearly

Some clearer signs, which can often be detected using home blood glucose monitors or urine ketone test kits, include:

  • High blood sugar levels

  • Elevated ketone levels in urine

When to See a Doctor

If you’re feeling unwell, under stress, or recovering from a recent illness or injury, it’s important to monitor your blood sugar levels closely. You can also use urine ketone test strips, available at pharmacies, to check ketone levels at home.

Contact your healthcare provider immediately if:

  • You’re vomiting and unable to keep food or liquids down

  • Your blood sugar remains above your target range and doesn’t improve with home treatments

  • Your urine ketone level is moderate to high

Seek emergency medical care if:

  • Your blood sugar level stays above 300 mg/dL (16.7 mmol/L) on repeated tests

  • You have ketones in your urine and can’t reach your doctor

  • You experience multiple symptoms of diabetic ketoacidosis, such as severe thirst, frequent urination, nausea, vomiting, stomach pain, extreme weakness, shortness of breath, fruity-smelling breath, or confusion

Remember: Untreated diabetic ketoacidosis can become life-threatening.

Causes

Glucose (sugar) is the body’s primary source of energy for muscles and other tissues. Insulin plays a critical role in allowing glucose to enter the body’s cells.

When there isn’t enough insulin, the body cannot use glucose for energy. In response, the body releases hormones that cause fat to break down for fuel, producing acids called ketones. These ketones accumulate in the blood and eventually spill into the urine.

Common causes of diabetic ketoacidosis include:

  • Illness or infection: Conditions like pneumonia or urinary tract infections can trigger the release of hormones that counteract insulin’s effect, increasing the risk of DKA.

  • Problems with insulin therapy: Missing insulin doses, using too little insulin, or issues with an insulin pump can lead to dangerously low insulin levels.

  • Other potential triggers:

    • Physical or emotional stress

    • Heart attack or stroke

    • Inflammation of the pancreas (pancreatitis)

    • Pregnancy

    • Alcohol or drug abuse, especially cocaine

    • Certain medications like corticosteroids and some diuretics

ডায়াবেটিক কিটোঅ্যাসিডোসিসের কারণসমূহ

গ্লুকোজ (চিনি) হলো শরীরের পেশি ও অন্যান্য টিস্যুর প্রধান শক্তির উৎস। ইনসুলিন শরীরের কোষে গ্লুকোজ প্রবেশ করতে সাহায্য করে।

যখন শরীরে পর্যাপ্ত ইনসুলিন থাকে না, তখন গ্লুকোজ শক্তির জন্য ব্যবহার করা যায় না। ফলে শরীর বিকল্প শক্তি উৎস হিসেবে চর্বি ভাঙতে শুরু করে। এর ফলে কিটোন নামক অ্যাসিড তৈরি হয়। এই কিটোন রক্তে জমে ওঠে এবং পরে প্রস্রাবের মাধ্যমে বের হয়।
