Delirium

Delirium (ডিলিরিয়াম ) is a serious change in mental function that causes confusion and reduced awareness of the surroundings. It develops rapidly, often within hours or a few days.

Delirium can be triggered by various factors such as severe or prolonged illness, chemical imbalances like low sodium levels, certain medications, infections, surgery, or the use or withdrawal of alcohol and drugs.

The symptoms of delirium can sometimes be mistaken for those of dementia. Doctors often depend on observations and information from family members or caregivers to help make an accurate diagnosis.

ডিলিরিয়াম একটি গুরুতর মানসিক পরিবর্তন যা মানুষের চিন্তাভাবনায় বিভ্রান্তি এবং আশেপাশের পরিবেশ সম্পর্কে সচেতনতা কমিয়ে দেয়। এটি দ্রুত উন্নতি ঘটে, সাধারণত কয়েক ঘণ্টা থেকে কয়েক দিনের মধ্যে।

ডিলিরিয়ামের কারণ বিভিন্ন হতে পারে, যেমন গুরুতর বা দীর্ঘস্থায়ী রোগ, শরীরের রসায়নিক ভারসাম্যহীনতা যেমন সোডিয়ামের ঘাটতি, কিছু ওষুধ, সংক্রমণ, অস্ত্রোপচার, অথবা মাদক বা মদ্যপানের ব্যবহার বা বিরতি।

ডিলিরিয়ামের লক্ষণগুলো কখনো কখনো ডিমেনশিয়ার লক্ষণের সঙ্গে মিল থাকতে পারে। সঠিক নির্ণয়ের জন্য ডাক্তাররা প্রায়ই পরিবারের সদস্য বা যত্নশীলের কাছ থেকে তথ্য নেন।

Symptoms

Delirium symptoms usually develop over a few hours or days and often accompany an underlying medical issue. These symptoms can fluctuate throughout the day, sometimes disappearing entirely. They often worsen at night or in unfamiliar places, such as hospitals.

Common symptoms include:

  • Reduced awareness of surroundings, which may cause difficulty focusing, getting stuck on certain thoughts, being easily distracted, withdrawing from activity, or having little response to the environment.

  • Poor thinking skills, such as memory loss (forgetting recent events), confusion about location or identity, difficulty speaking or finding words, rambling speech, trouble understanding others, or problems with reading and writing.

  • Changes in behavior and emotions, including anxiety, fear, distrust, depression, irritability, mood swings, personality shifts, hallucinations, restlessness, calling out, quiet withdrawal (especially in older adults), slow movements, and altered sleep patterns like a reversed day-night cycle.

Types of Delirium

There are three recognized types:

  1. Hyperactive delirium – characterized by restlessness, pacing, anxiety, rapid mood changes, hallucinations, and resistance to care.

  2. Hypoactive delirium – marked by inactivity, sluggishness, drowsiness, and reduced interaction with others.

  3. Mixed delirium – involves symptoms of both hyperactive and hypoactive types, with the person switching between restlessness and sluggishness.

Delirium vs. Dementia

Though delirium and dementia share some symptoms, they are different conditions and can coexist. Dementia progresses slowly, causing a steady decline in memory and thinking over months or years, with Alzheimer’s disease being the most common cause.

Key differences:

  • Delirium begins quickly (within days), while dementia develops gradually.

  • Delirium affects attention and focus severely, but early dementia patients remain more alert.

  • Delirium symptoms fluctuate during the day, while dementia symptoms are more constant.

When to See a Doctor

If you notice delirium symptoms in a loved one or someone you care for, inform their healthcare provider. Your observations about their normal behavior and current changes are vital for accurate diagnosis and treatment. In hospitals or care facilities, alert staff if symptoms appear, as delirium is common among older patients and those seriously ill.

Causes of Delirium

Delirium happens when the brain’s communication signals are disrupted. It may result from a single cause or a combination of factors, including:

  • Side effects of certain medications

  • Alcohol or drug use or withdrawal

  • Medical problems such as stroke, heart attack, lung or liver disease, or injury

  • Imbalances in body chemicals like sodium or calcium

  • Severe or prolonged illness

  • Fever or infections, especially in children and older adults (e.g., urinary tract infections, pneumonia, flu, COVID-19)

  • Exposure to toxins like carbon monoxide or poisons

  • Poor nutrition or dehydration

  • Lack of sleep or extreme emotional stress

  • Pain

  • Surgery or medical procedures requiring anesthesia

Certain medicines alone or combined may trigger delirium. These include drugs for pain, sleep disorders, mood disorders, allergies, asthma, swelling, Parkinson’s disease, or seizures.

ডিলিরিয়ামের লক্ষণ সাধারণত কয়েক ঘণ্টা থেকে কয়েক দিনের মধ্যে শুরু হয় এবং প্রায়ই কোনো গুরুতর শারীরিক সমস্যার সঙ্গে যুক্ত থাকে। লক্ষণগুলো দিনের মধ্যে উঠানামা করে, কখনো কখনো পুরোপুরি চলে যেতে পারে। রাতের বেলা বা অপরিচিত পরিবেশে, যেমন হাসপাতাল, এসব লক্ষণ আরও বেশি হয়।

প্রধান লক্ষণগুলো হল:

  • পরিবেশ সম্পর্কে সচেতনতার কমে যাওয়া, যার ফলে মনোযোগ দিতে কষ্ট হওয়া, কোনো বিষয়ে আটকে যাওয়া, সহজে বিভ্রান্ত হওয়া, অনাগ্রহ বা পরিবেশের প্রতি কম সাড়া দেওয়া।

  • চিন্তার দক্ষতার হ্রাস, যেমন সাম্প্রতিক ঘটনা ভুলে যাওয়া, নিজের অবস্থান বা পরিচয় ভুলে যাওয়া, কথা বলা বা শব্দ খোঁজার সমস্যা, অসংলগ্ন কথা বলা, অন্যদের কথা বোঝার সমস্যা, পড়া-লেখায় অসুবিধা।

  • আচরণ ও আবেগে পরিবর্তন, যেমন উদ্বেগ, ভয়, অবিশ্বাস, বিষণ্ণতা, রাগ, মানসিক উৎফুল্লতা, আগ্রহ ও আবেগহীনতা, দ্রুত মেজাজ বদল, ব্যক্তিত্ব পরিবর্তন, ভ্রান্তি দেখা, চঞ্চলতা, শাঁখা আওয়াজ করা, নীরব থাকা (বিশেষ করে বয়স্কদের মধ্যে), ধীরগতি, ঘুমের অভ্যাস পরিবর্তন যেমন দিন-রাত পাল্টে যাওয়া।

ডিলিরিয়ামের ধরণ

ডিলিরিয়ামের তিনটি ধরন আছে:

১. হাইপারঅ্যাকটিভ ডিলিরিয়াম – চঞ্চলতা, হাঁটাহাঁটি, উদ্বেগ, দ্রুত মেজাজ বদল, ভ্রান্তি দেখা, চিকিৎসার বিরুদ্ধে প্রতিরোধ।
২. হাইপোঅ্যাকটিভ ডিলিরিয়াম – নিষ্ক্রিয়তা, ঢিমেতি ভাব, ঘুমালু অবস্থা, পরিবারের সঙ্গে কম যোগাযোগ।
৩. মিক্সড ডিলিরিয়াম – উপরের দুটির সংমিশ্রণ, যেখানে ব্যক্তি দ্রুত চঞ্চল থেকে ধীরগতি অবস্থায় পরিবর্তিত হয়।

ডিলিরিয়াম ও ডিমেনশিয়ার পার্থক্য

ডিলিরিয়াম ও ডিমেনশিয়া কিছু লক্ষণে মিল থাকা সত্ত্বেও ভিন্ন অবস্থা এবং একসঙ্গে থাকতে পারে। ডিমেনশিয়া ধীরে ধীরে স্মৃতি ও চিন্তাশক্তির অবনতি ঘটায়, যার সবচেয়ে সাধারণ কারণ আলঝেইমার্স রোগ।

প্রধান পার্থক্য:

  • ডিলিরিয়াম দ্রুত (দিনের মধ্যে) শুরু হয়, ডিমেনশিয়া ধীরে ধীরে।

  • ডিলিরিয়ামে মনোযোগ ও ফোকাস খুব কমে যায়, ডিমেনশিয়ার প্রথম দিকে রোগী সচেতন থাকে।

  • ডিলিরিয়ামের লক্ষণ দিনের মধ্যে ওঠানামা করে, ডিমেনশিয়ার লক্ষণ সাধারণত স্থিতিশীল থাকে।

কখন ডাক্তারকে দেখাবেন

আপনার পরিচিত বা যত্ন নেওয়া কারো মধ্যে ডিলিরিয়ামের লক্ষণ দেখা দিলে তাদের স্বাস্থ্যপরিচর্যা প্রদানকারীর সঙ্গে কথা বলুন। তাদের স্বাভাবিক আচরণ এবং বর্তমান পরিবর্তনগুলি জানানো সঠিক নির্ণয়ের জন্য জরুরি। হাসপাতাল বা নার্সিং হোমে থাকলে চিকিৎসা কর্মীদের জানান, কারণ বয়স্ক ও গুরুতর অসুস্থ ব্যক্তিদের মধ্যে ডিলিরিয়াম সাধারণ।

ডিলিরিয়ামের কারণ

মস্তিষ্কের সংকেত সঠিকভাবে না পৌঁছানো ও গ্রহণের কারণে ডিলিরিয়াম হয়। একাধিক কারণ একসঙ্গে থাকতে পারে, যেমন:

  • কিছু ওষুধ বা তাদের পার্শ্বপ্রতিক্রিয়া

  • মদ বা মাদক ব্যবহারের বা ত্যাগের ফলে

  • স্ট্রোক, হার্ট অ্যাটাক, ফুসফুস বা লিভারের অসুখ, আঘাত

  • শরীরের সোডিয়াম বা ক্যালসিয়ামের ঘাটতি

  • দীর্ঘস্থায়ী বা গুরুতর রোগ

  • জ্বর ও নতুন সংক্রমণ, বিশেষ করে শিশু ও বয়স্কদের ক্ষেত্রে (যেমন ইউরিনারি ট্র্যাক্ট ইনফেকশন, নিউমোনিয়া, ফ্লু, কোভিড-১৯)

  • বিষাক্ত পদার্থের সংস্পর্শ (কার্বন মনোক্সাইড, সায়নাইড)

  • খারাপ পুষ্টি বা শরীর থেকে বেশি পানি হারানো

  • ঘুমের অভাব বা মানসিক চাপ

  • ব্যথা

  • অস্ত্রোপচার বা অ্যানাস্থেসিয়ার প্রয়োজন এমন চিকিৎসা

কিছু ওষুধ একাকী বা একসঙ্গে ডিলিরিয়াম সৃষ্টি করতে পারে, যেমন ব্যথার ওষুধ, ঘুমের ওষুধ, মানসিক রোগের ওষুধ, অ্যালার্জি, অ্যাজমা, ফোলাভাব, পারকিনসন রোগ, অথবা মৃগীর ওষুধ।
