Cryoglobulinemia

Cryoglobulinemia (ক্রায়োগ্লোবুলিনেমিয়া ) is a group of rare medical conditions that fall under vasculitis — a disorder that leads to inflammation of the blood vessels. This inflammation can restrict or disrupt blood flow, potentially damaging various parts of the body.

In individuals with cryoglobulinemia, abnormal proteins called cryoglobulins are found in the bloodstream. These proteins can stick together and form clumps when exposed to temperatures below normal body heat (98.6°F or 37°C). The clumping of these proteins may obstruct blood circulation, resulting in injury to the skin, joints, nerves, and vital organs such as the kidneys and liver.

ক্রায়োগ্লোবুলিনেমিয়া একটি বিরল রোগের গ্রুপ, যা ভাসকুলাইটিস নামক অবস্থার অন্তর্গত। ভাসকুলাইটিস হলো এক ধরনের রোগ যা রক্তনালির প্রদাহ বা ফোলাভাব সৃষ্টি করে। এই প্রদাহ রক্তপ্রবাহ ব্যাহত করতে পারে, ফলে শরীরের বিভিন্ন অংশে ক্ষতি হতে পারে।

এই রোগে আক্রান্ত ব্যক্তিদের রক্তে কিছু অস্বাভাবিক প্রোটিন থাকে, যাদের বলা হয় ক্রায়োগ্লোবুলিন। শরীরের স্বাভাবিক তাপমাত্রার নিচে (৯৮.৬°F বা ৩৭°C) নামলে এই প্রোটিনগুলো একত্রিত হয়ে দলা পাকাতে পারে। এই প্রোটিনের দলা রক্তনালিতে জমে রক্তপ্রবাহ বন্ধ করে দিতে পারে, যার ফলে ত্বক, জয়েন্ট, স্নায়ু এবং গুরুত্বপূর্ণ অঙ্গ যেমন কিডনি ও লিভার ক্ষতিগ্রস্ত হতে পারে।

Symptoms:

Some people with cryoglobulinemia do not show any symptoms. For others, the symptoms may appear and disappear over time. Common symptoms include:

  • Skin Spots: Most individuals develop purplish patches or spots (known as lesions) on their legs. On darker skin tones, these may appear as dark brown or black. Some people may also experience open wounds or ulcers on the legs.

  • Joint Pain: Many people experience joint pain similar to symptoms of rheumatoid arthritis, affecting mobility and comfort.

  • Peripheral Neuropathy: Damage to the nerves in the fingers and toes can occur, leading to numbness, tingling, or weakness in those areas.

Causes

The exact cause of cryoglobulinemia is not always known. However, it is often associated with the following conditions:

  • Infections: Chronic infections, especially Hepatitis C, are strongly linked to cryoglobulinemia. Other infections that may contribute include Hepatitis B, HIV, Epstein-Barr virus, toxoplasmosis, and malaria.

  • Certain Blood Cancers: Conditions such as multiple myeloma, Waldenström macroglobulinemia, and chronic lymphocytic leukemia can be associated with cryoglobulinemia.

  • Autoimmune Diseases: Diseases in which the immune system mistakenly attacks healthy cells can increase the risk. Examples include lupus, rheumatoid arthritis, and Sjogren’s syndrome.

সব সময় এই রোগের কোনো উপসর্গ নাও থাকতে পারে। তবে যাদের উপসর্গ থাকে, তাদের ক্ষেত্রে সেগুলো মাঝে মাঝে দেখা দিতে পারে এবং আবার চলে যেতে পারে। সাধারণ উপসর্গগুলোর মধ্যে রয়েছে:

  • ত্বকে দাগ: অধিকাংশ রোগীর পায়ে বেগুনি রঙের দাগ (লেজন) দেখা যায়। গা dark ় রঙের ত্বকে এটি কালচে বা বাদামি রঙের দেখাতে পারে। কারো কারো ক্ষেত্রে পায়ে খোলা ঘা (আলসার) ও হতে পারে।

  • জয়েন্টে ব্যথা: রিউমাটয়েড আর্থ্রাইটিসের মত জয়েন্ট ব্যথা এই রোগে সাধারণ একটি উপসর্গ।

  • পেরিফেরাল নিউরোপ্যাথি: আঙুল ও পায়ের আঙুলের প্রান্তের স্নায়ু ক্ষতিগ্রস্ত হলে অসাড়তা, ঝিনঝিন অনুভব বা দুর্বলতা দেখা দিতে পারে।

ক্রায়োগ্লোবুলিনেমিয়ার কারণ

এই রোগটি কীভাবে হয়, তা সবসময় পরিষ্কার নয়। তবে এটি কিছু শারীরিক অবস্থার সঙ্গে সম্পর্কিত হতে পারে:

  • সংক্রমণ: হেপাটাইটিস C ভাইরাস সংক্রমণ এই রোগের সঙ্গে সবচেয়ে বেশি জড়িত। এছাড়া হেপাটাইটিস B, এইচআইভি (HIV), এপস্টিন-বার ভাইরাস, টক্সোপ্লাজমোসিস এবং ম্যালেরিয়া-র মতো সংক্রমণও ভূমিকা রাখতে পারে।

  • রক্তের কিছু ধরনের ক্যান্সার: যেমন মাল্টিপল মাইলোমা, ওয়ালডেনস্ট্রম ম্যাক্রোগ্লোবুলিনেমিয়া এবং ক্রনিক লিম্ফোসাইটিক লিউকেমিয়া।

  • অটোইমিউন রোগ: যেসব রোগে ইমিউন সিস্টেম শরীরের নিজের কোষের ওপর আক্রমণ করে, যেমন লুপাস, রিউমাটয়েড আর্থ্রাইটিস এবং শজোগ্রেন সিনড্রোম, সেগুলো এই রোগের ঝুঁকি বাড়ায়।
