Symptoms
Common symptoms of Crohn’s disease include:
-
Diarrhea
-
Fever
-
Fatigue
-
Abdominal pain and cramps
-
Blood in the stool
-
Mouth ulcers
-
Loss of appetite and weight loss
-
Pain or discharge near the anus caused by inflammation from a tunnel in the skin, known as a fistula
Crohn’s disease can affect any part of the small or large intestine. It may affect multiple segments or be continuous, but it most often targets the last part of the small intestine. In some cases, the disease affects only the colon (large intestine).
Symptoms may range from mild to severe and usually develop gradually. However, sometimes symptoms may appear suddenly without warning. People with Crohn’s disease can also experience symptom-free periods, called remission.
Other possible symptoms in severe cases include:
-
Inflammation of the skin, eyes, or joints
-
Inflammation of the liver or bile ducts
-
Kidney stones
-
Iron deficiency (anemia)
-
Delayed growth or sexual development in children

Crohn’s Disease: Causes
The exact cause of Crohn’s disease is not fully understood. Earlier, diet and stress were thought to cause the disease, but now experts know these may worsen symptoms rather than cause the condition. Multiple factors are believed to contribute to its development:
-
Genetics: More than 200 genes are linked to Crohn’s disease. While it’s unclear how these genes affect the disease, having certain genetic markers may increase the risk.
-
Immune system: Certain bacteria, viruses, or environmental triggers may activate the immune system abnormally. The immune system mistakenly attacks the digestive tract, causing inflammation. Some gut bacteria are suspected to be involved, but their exact role is still unknown.

Diagnosis
A healthcare provider typically diagnoses Crohn’s disease after eliminating other possible causes of symptoms, as there is no single definitive test for this condition.
Several tests are usually combined to confirm a diagnosis of Crohn’s disease, including:
Laboratory Tests
-
Blood Tests: These tests check for signs of infection or anemia, a condition where there are not enough red blood cells to carry oxygen efficiently to body tissues. Blood tests can also assess levels of inflammation, liver function, and detect dormant infections like tuberculosis. Additionally, they may test for immunity to certain infections.
-
Stool Tests: A stool sample can be analyzed to detect blood, bacteria, or parasites that may cause diarrhea or other symptoms. Markers of inflammation in the stool, such as calprotectin, can also help identify intestinal inflammation.
Procedures
-
Colonoscopy: This procedure uses a flexible tube with a camera to examine the entire colon and the end of the small intestine (ileum). Small tissue samples (biopsies) may be taken to look for inflammatory cells called granulomas, which can suggest Crohn’s disease.
-
CT Scan: A computed tomography (CT) scan offers detailed images of the bowel and surrounding tissues. CT enterography, a specialized CT scan with oral and intravenous contrast, provides detailed views of the small intestine and has replaced some traditional X-rays.
-
MRI: Magnetic resonance imaging (MRI), especially MR enterography with contrast, is useful for evaluating the intestines and areas around the anus, such as fistulas. This imaging method is often used to monitor disease progression and is preferred for younger patients due to the absence of radiation.
-
Capsule Endoscopy: In this test, the patient swallows a small camera capsule that captures images of the small intestine. The images are transmitted to a recorder worn on the body and later reviewed by the doctor. This procedure helps detect Crohn’s disease signs but is avoided if bowel narrowing or blockage is suspected.
Treatment
There is no cure for Crohn’s disease, and treatment varies for each person. The primary goals are to reduce inflammation, relieve symptoms, and prevent complications. Effective treatment can sometimes lead to long-term remission.
Anti-inflammatory Medications
-
Corticosteroids: Drugs like prednisone and budesonide reduce inflammation and may be used short term to induce remission. They are sometimes combined with immune suppressors and then gradually discontinued.
-
Oral 5-Aminosalicylates: These medications, such as sulfasalazine and mesalamine, are more effective for inflammation in the colon than in the small intestine and are used in mild to moderate cases.
Immune System Suppressors
These medicines decrease inflammation by targeting the immune system:
-
Azathioprine and Mercaptopurine: Widely used immunosuppressants requiring regular blood monitoring due to possible side effects like infection risk and liver inflammation.
-
Methotrexate: Used when other treatments fail; requires close monitoring.
Biologic Therapies
Biologics target specific immune proteins involved in inflammation:
-
TNF Inhibitors: Include infliximab, adalimumab, and certolizumab pegol, which block tumor necrosis factor, a protein that promotes inflammation.
-
Other Biologics: Ustekinumab and vedolizumab target different inflammatory pathways, with vedolizumab being gut-specific. Risankizumab, a newer option, targets interleukin-23 for moderate to severe disease.
Janus Kinase (JAK) Inhibitors
These oral medications block immune pathways that cause inflammation and are used when other treatments are ineffective. They are not recommended during pregnancy.
Antibiotics
Antibiotics like ciprofloxacin and metronidazole may help control fistulas and abscesses and reduce harmful gut bacteria.
Symptom Relief and Other Therapies
-
Anti-diarrheals and Fiber Supplements: Used cautiously to manage diarrhea.
-
Pain Relievers: Acetaminophen is preferred; NSAIDs such as ibuprofen should be avoided as they may worsen symptoms.
-
Nutritional Support: Vitamins and supplements may be necessary if nutrient absorption is impaired.
Nutrition Therapy
Special diets or feeding through tubes may be used to improve nutrition and allow the bowel to rest. Parenteral nutrition (intravenous feeding) may be needed before surgery or if medicines fail.
Surgery
If medications and diet do not control symptoms, surgery may be required to remove damaged bowel sections or treat complications like fistulas and abscesses. Surgery is not a cure; disease often recurs near surgical sites, so ongoing medical treatment is important.
