Cholera

Cholera(কলেরা) is a bacterial infection that spreads mainly through contaminated water. It leads to severe diarrhea and rapid dehydration. Without prompt treatment, cholera can become life-threatening within hours, even in otherwise healthy individuals.

Thanks to modern sewage systems and clean water supplies, cholera is rare in developed countries. However, it remains a concern in parts of Africa, Southeast Asia, and Haiti. The risk of outbreaks is higher in areas affected by poverty, natural disasters, or conflict, where access to clean water and proper sanitation is limited.

Cholera is highly treatable. With timely use of an affordable and simple rehydration solution, most deaths from severe dehydration can be prevented.

কলেরা একটি ব্যাকটেরিয়া সংক্রমণ, যা সাধারণত দূষিত পানির মাধ্যমে ছড়িয়ে পড়ে। এটি মারাত্মক ডায়রিয়া এবং দ্রুত পানিশূন্যতা সৃষ্টি করে। সঠিক চিকিৎসা ছাড়া, কলেরা কয়েক ঘণ্টার মধ্যেই প্রাণঘাতী হতে পারে, এমনকি আগে সুস্থ থাকা ব্যক্তিদের ক্ষেত্রেও।

আধুনিক পয়ঃনিষ্কাশন ব্যবস্থা এবং নিরাপদ পানির সহজলভ্যতার কারণে উন্নত দেশগুলোতে কলেরা এখন খুবই দুর্লভ। তবে আফ্রিকা, দক্ষিণ-পূর্ব এশিয়া এবং হাইতিতে এটি এখনও একটি বড় সমস্যা। যেখানে দারিদ্র্য, প্রাকৃতিক দুর্যোগ বা যুদ্ধের কারণে মানুষ অপর্যাপ্ত স্যানিটেশন এবং নিরাপদ পানির অভাবে ঘনবসতিপূর্ণ অবস্থায় বসবাস করে, সেখানে কলেরার প্রাদুর্ভাবের ঝুঁকি বেশি।

কলেরার চিকিৎসা সহজ এবং কার্যকর। সময়মতো একটি সহজ ও সাশ্রয়ী রিহাইড্রেশন সলিউশন ব্যবহার করে মারাত্মক পানিশূন্যতার কারণে মৃত্যুর ঝুঁকি অনেকাংশে কমানো যায়।

Symptoms

Many individuals exposed to the cholera bacteria (Vibrio cholerae) may not experience any symptoms and may not even realize they are infected. However, they can still spread the bacteria to others through contaminated water, as they shed the bacteria in their stool for up to 7 to 14 days.

When symptoms do occur, they are often mild or moderate and can resemble other causes of diarrhea. In more serious cases, symptoms usually develop within a few days after infection.

Common symptoms of cholera include:

  • Diarrhea: Cholera-induced diarrhea often starts suddenly and can cause severe fluid loss — up to one liter per hour. The stool typically appears pale and milky, resembling rice-water.

  • Nausea and vomiting: Vomiting is common during the early stages of the infection and may last for several hours.

  • Dehydration: Dehydration can occur rapidly after symptoms begin, ranging from mild to severe. Losing 10% or more of body weight indicates severe dehydration.

Signs of dehydration include irritability, fatigue, sunken eyes, dry mouth, intense thirst, dry and wrinkled skin, reduced or no urination, low blood pressure, and irregular heartbeat.

Electrolyte Imbalance

Severe dehydration can lead to an imbalance of minerals (electrolytes) in the blood, resulting in complications such as:

  • Muscle cramps: Caused by the sudden loss of salts like sodium, chloride, and potassium.

  • Shock: A critical condition resulting from low blood volume, causing dangerously low blood pressure and oxygen levels. Without immediate treatment, hypovolemic shock can be fatal within minutes.

When to See a Doctor

Cholera is rare in industrialized countries. However, if you develop severe diarrhea after traveling to a region with ongoing cholera outbreaks, seek medical attention immediately.

Prompt treatment is crucial if you experience diarrhea, especially if it’s severe, to prevent life-threatening dehydration.

Causes

Cholera is caused by the bacterium Vibrio cholerae. The bacteria release a toxin in the small intestine that forces the body to expel large amounts of water, leading to diarrhea, dehydration, and electrolyte loss.

While not everyone exposed to the bacteria becomes ill, they can still carry and spread the infection through contaminated food and water.

The main sources of cholera infection include:

  • Contaminated water: Drinking or using contaminated surface water or public well water is a leading cause, especially in areas lacking proper sanitation.

  • Seafood: Consuming raw or undercooked seafood, particularly shellfish from contaminated waters, can transmit cholera. In the United States, some cases have been linked to seafood from the Gulf of Mexico.

  • Raw fruits and vegetables: Eating unwashed or unpeeled produce in areas where cholera is present can be risky. Crops irrigated with contaminated water or fertilized with untreated manure may carry the bacteria.

  • Grains: Cooked grains like rice and millet, if left at room temperature for extended periods, can become breeding grounds for cholera bacteria in affected regions.

কলেরার লক্ষণসমূহ

অনেক মানুষ যারা কলেরা জীবাণু (Vibrio cholerae) দ্বারা সংক্রমিত হন, তারা কোনো উপসর্গ অনুভব করেন না এবং বুঝতেও পারেন না যে তারা সংক্রমিত হয়েছেন। তবে তারা মলত্যাগের মাধ্যমে ৭ থেকে ১৪ দিন পর্যন্ত জীবাণু ছড়াতে পারেন, যা অন্যদের সংক্রমিত করতে পারে।

যাদের উপসর্গ দেখা দেয়, তাদের ক্ষেত্রে তা সাধারণত মৃদু বা মাঝারি মাত্রার হয় এবং অন্যান্য ডায়রিয়ার সাথে বিভ্রান্তিকর হতে পারে। গুরুতর ক্ষেত্রে, সংক্রমণের কয়েক দিনের মধ্যেই লক্ষণ প্রকাশ পায়।

কলেরা সংক্রমণের সাধারণ লক্ষণগুলো হলো:

  • ডায়রিয়া: কলেরার কারণে ডায়রিয়া হঠাৎ শুরু হয় এবং খুব দ্রুত বিপজ্জনক মাত্রায় তরল ক্ষতি ঘটাতে পারে — প্রতি ঘণ্টায় প্রায় ১ লিটার পর্যন্ত। মলের রঙ সাধারণত ধোঁয়াটে সাদা হয়, যা ধুয়ে নেওয়া চালের পানির মতো দেখায়।

  • বমি বমি ভাব ও বমি: সংক্রমণের শুরুতে বমি হওয়া স্বাভাবিক এবং এটি কয়েক ঘণ্টা পর্যন্ত স্থায়ী হতে পারে।

  • পানিশূন্যতা: ডায়রিয়া শুরুর কয়েক ঘণ্টার মধ্যেই পানিশূন্যতা হতে পারে। শরীরের ওজনের ১০% বা তার বেশি হ্রাস হলে তা গুরুতর পানিশূন্যতার ইঙ্গিত দেয়।

পানিশূন্যতার লক্ষণসমূহের মধ্যে রয়েছে: উত্তেজনা, ক্লান্তি, চোখ বসে যাওয়া, মুখ শুকিয়ে যাওয়া, তীব্র পিপাসা, কুঁচকানো ও ধীরগতিতে স্বাভাবিক অবস্থায় ফেরত আসা ত্বক, কম বা মূত্রত্যাগ না হওয়া, নিম্ন রক্তচাপ এবং অনিয়মিত হৃদস্পন্দন।

ইলেকট্রোলাইট ভারসাম্যহীনতা

চরম পানিশূন্যতার কারণে শরীরের রক্তের ইলেকট্রোলাইটের ভারসাম্য নষ্ট হতে পারে, যার ফলে দেখা দিতে পারে:

  • পেশির খিঁচুনি: সোডিয়াম, ক্লোরাইড ও পটাসিয়ামের মতো লবণের দ্রুত ক্ষতির কারণে খিঁচুনি হয়।

  • শক (Shock): নিম্ন রক্তচাপ এবং শরীরে অক্সিজেনের ঘাটতির কারণে ঘটে। এটি চিকিৎসা না পেলে কয়েক মিনিটের মধ্যেই প্রাণঘাতী হতে পারে।

কখন চিকিৎসকের পরামর্শ নেওয়া উচিত

উন্নত দেশগুলোতে কলেরার ঝুঁকি খুবই কম। তবে কলেরা আক্রান্ত অঞ্চলে ভ্রমণের পর যদি তীব্র ডায়রিয়া দেখা দেয়, তাহলে দ্রুত চিকিৎসকের পরামর্শ নেওয়া জরুরি।

বিশেষ করে যদি ডায়রিয়া গুরুতর হয়, তাহলে তাৎক্ষণিক চিকিৎসা নেওয়া জরুরি, কারণ চরম পানিশূন্যতা জীবনহানির কারণ হতে পারে।

কলেরার কারণসমূহ

কলেরা রোগের কারণ হলো Vibrio cholerae নামক ব্যাকটেরিয়া। এটি অন্ত্রের ভেতরে একটি বিষাক্ত পদার্থ (টক্সিন) উৎপন্ন করে, যা শরীর থেকে প্রচুর পরিমাণে পানি বের করে দেয়, ফলে ডায়রিয়া, পানিশূন্যতা এবং ইলেকট্রোলাইটের ভারসাম্যহীনতা দেখা দেয়।

সব মানুষ আক্রান্ত হলে অসুস্থ হয় না, তবে তারা সংক্রমণ ছড়াতে পারে।

কলেরা সংক্রমণের প্রধান উৎসসমূহ হলো:

  • দূষিত পানি: দূষিত পৃষ্ঠজল বা কূপের পানি পান করলে কলেরা সংক্রমণের ঝুঁকি বেড়ে যায়, বিশেষ করে যেখানে সঠিক স্যানিটেশন ব্যবস্থা নেই।

  • সামুদ্রিক খাবার: কাঁচা বা অপর্যাপ্ত রান্না করা সামুদ্রিক খাবার, বিশেষ করে ঝিনুকজাতীয় খাবার খেলে সংক্রমণ হতে পারে। যুক্তরাষ্ট্রে কলেরা সংক্রমণের কিছু ঘটনা মেক্সিকো উপসাগরের সামুদ্রিক খাবারের সাথে যুক্ত।

  • কাঁচা ফল ও শাকসবজি: কলেরা আক্রান্ত অঞ্চলে অমোছা বা অপরিষ্কৃত ফল ও সবজি খাওয়ার মাধ্যমে সংক্রমণের ঝুঁকি থাকে। অপরিষ্কৃত মল সার বা দূষিত সেচের পানি দিয়ে চাষ করা শাকসবজিও বিপজ্জনক।

  • শস্যজাতীয় খাদ্য: কলেরা আক্রান্ত অঞ্চলে রান্না করার পর ঘরের তাপমাত্রায় দীর্ঘ সময় ফেলে রাখা চাল ও মিলেটের মতো শস্য খাবারে ব্যাকটেরিয়া বেড়ে উঠতে পারে।
