Chagas Disease

Chagas disease(চাগাস ডিজিজ) is an infection caused by the parasite Trypanosoma cruzi. This parasite is primarily found in the feces of triatomine bugs, also known as “kissing bugs” due to their tendency to bite the faces of people. These bugs are common in South America, Central America, and Mexico, though cases are occasionally reported in the southern United States.

Also referred to as American trypanosomiasis, Chagas disease can affect individuals of all ages. If left untreated, it can lead to severe complications, including heart and digestive issues.

In the early stages of infection, treatment focuses on eliminating the parasite. However, once the infection progresses, it’s no longer possible to completely eradicate the parasite. In such cases, treatment shifts to symptom management. Preventative measures are available to reduce the risk of infection.

চাগাস রোগ হল একটি সংক্রমণ যা ট্রিপানোসোমা ক্রুজি পরজীবী দ্বারা সৃষ্ট। এই পরজীবী সাধারণত ত্রায়াটোমিন পোকামাকড়ের মলমূত্রে থাকে, যেগুলিকে “কিসিং বাগ” বলা হয়, কারণ তারা প্রায়ই মানুষের মুখে কামড় দিয়ে থাকে। এই পোকামাকড়গুলি দক্ষিণ আমেরিকা, মধ্য আমেরিকা এবং মেক্সিকোতে সাধারণ, তবে বিরলভাবে, যুক্তরাষ্ট্রের দক্ষিণাঞ্চলেও এর প্রকোপ দেখা যায়।

সংক্রমণের প্রথম পর্যায়ে, চিকিৎসা মূলত পরজীবীটি নির্মূল করার দিকে মনোনিবেশ করে। তবে, একবার সংক্রমণ বাড়লে, পরজীবীটি পুরোপুরি নির্মূল করা আর সম্ভব হয় না। এই অবস্থায়, চিকিৎসা মূলত উপসর্গ ব্যবস্থাপনার দিকে পরিবর্তিত হয়। সংক্রমণ প্রতিরোধে কিছু উপায় রয়েছে যা এর ঝুঁকি কমাতে সাহায্য করতে পারে।

Symptoms:

Chagas disease can present itself as a sudden, short-term illness (acute phase) or as a long-term, chronic condition. Both stages may be symptom-free, but in some cases, the disease can cause life-threatening symptoms.

Acute Phase

The acute phase of Chagas disease lasts for a few weeks or months and often has no noticeable symptoms. When symptoms do occur, they are generally mild and may include:

  • Swelling at the site of infection

  • Fever

  • Fatigue

  • Rash

  • Muscle aches

  • Swelling of the eyelids

  • Headache

  • Loss of appetite

  • Nausea, diarrhea, or vomiting

  • Swollen lymph nodes

  • Enlarged liver or spleen

In most cases, symptoms in the acute phase subside on their own. However, if left untreated, the infection can persist in the body and may eventually progress to the chronic phase.

Chronic Phase

The chronic phase of Chagas disease can develop 10 to 20 years after initial infection. It may not show symptoms at first, but in severe cases, the disease can lead to:

  • Irregular heart rhythms

  • Heart failure

  • Sudden cardiac arrest

  • Difficulty swallowing

  • Stomach pain or constipation

Causes

Chagas disease is caused by the parasite Trypanosoma cruzi, which spreads through the feces of the triatomine bug, also known as the “kissing bug.” The parasite can infect these bugs when they consume blood from an animal that carries the parasite.

Triatomine bugs are typically found in mud, thatch, or adobe huts in Mexico, South America, and Central America. During the day, they hide in cracks in walls or roofs and become active at night, often feeding on humans while they sleep.

After feeding, infected bugs defecate, leaving parasite-laden feces on the skin. The parasites can then enter the body through the eyes, mouth, cuts, or the bite wound itself. Scratching or rubbing the bite site, which often happens during sleep, can help the parasites enter the body. Once inside, the parasites multiply and spread.

Chagas disease cannot be transmitted by simply being around an infected person, but it can spread through:

  • Consuming uncooked food contaminated with bug feces.

  • Being born to a mother infected with the parasite.

  • Receiving infected blood or an organ transplant from a donor carrying the parasite.

  • Laboratory exposure to the parasite.

  • Spending time in areas where infected wildlife, like raccoons or opossums, are present.

চাগাস রোগ হঠাৎ এবং সংক্ষিপ্ত সময়ের রোগ (তীব্র পর্যায়) বা দীর্ঘস্থায়ী, দীর্ঘকালীন অবস্থার (দীর্ঘস্থায়ী পর্যায়) রূপে দেখা দিতে পারে। দুটি পর্যায়েই লক্ষণবিহীন হতে পারে, তবে কিছু ক্ষেত্রে এটি জীবন-সংকটজনক লক্ষণ সৃষ্টি করতে পারে।

তীব্র পর্যায়
চাগাস রোগের তীব্র পর্যায় কয়েক সপ্তাহ বা মাস স্থায়ী হতে পারে এবং প্রায়ই কোন লক্ষণ থাকে না। তবে, যখন লক্ষণ উপস্থিত হয়, তা সাধারণত হালকা এবং এর মধ্যে থাকতে পারে:

  • সংক্রমণের স্থানে ফুলে যাওয়া

  • জ্বর

  • ক্লান্তি

  • চুলকানি

  • শরীরব্যথা

  • চোখের পাতা ফুলে যাওয়া

  • মাথাব্যথা

  • ক্ষুধামন্দা

  • বমি বা ডায়রিয়া

  • ফোলা গ্রন্থি

  • বড় মেদ বা পিত্তযন্ত্র

তীব্র পর্যায়ে সাধারণত লক্ষণগুলি নিজে থেকেই চলে যায়। তবে, যদি সংক্রমণ চিকিৎসা না করা হয়, তবে এটি শরীরে থেকে যায় এবং পরে এটি দীর্ঘস্থায়ী পর্যায়ে চলে যেতে পারে।

দীর্ঘস্থায়ী পর্যায়
চাগাস রোগের দীর্ঘস্থায়ী পর্যায় ১০ থেকে ২০ বছর পর দেখা দিতে পারে। প্রথমে কোন লক্ষণ না-ও থাকতে পারে, তবে গুরুতর ক্ষেত্রে চাগাস রোগের লক্ষণগুলি অন্তর্ভুক্ত করতে পারে:

  • অনিয়মিত হৃদস্পন্দন

  • হৃদরোগ

  • আকস্মিক হৃদযন্ত্রের থামা

  • খাওয়ার সময় সমস্যা

  • পেট ব্যথা বা কোষ্ঠকাঠিন্য

চাগাস রোগের কারণ
চাগাস রোগ সৃষ্টিকারী পরজীবী ট্রিপানোসোমা ক্রুজি, যা ত্রায়াটোমিন পোকামাকড়ের মল থেকে ছড়ায়, যেগুলিকে “কিসিং বাগ” বলা হয়। এই পরজীবী পোকামাকড়গুলিকে সংক্রমিত করে যখন তারা এমন কোনো প্রাণীর রক্ত পান করে যা এই পরজীবী দ্বারা আক্রান্ত।

ত্রায়াটোমিন পোকামাকড়গুলি সাধারণত মেক্সিকো, দক্ষিণ আমেরিকা এবং মধ্য আমেরিকায় কাদামাটি, ছাউনি বা অ্যাডোবি কুঁড়েঘরে বাস করে। দিনের বেলায় তারা দেয়াল বা ছাদের ফাঁকে লুকিয়ে থাকে এবং রাতে সক্রিয় হয়ে মানুষের মুখে কামড় দেয়।

খাওয়ার পর, সংক্রমিত পোকামাকড় মলত্যাগ করে, যা ত্বকে পরজীবী রেখে দেয়। পরজীবীগুলি চোখ, মুখ, ক্ষত বা কামড়ের ঘা থেকে শরীরে প্রবেশ করতে পারে। কামড়ের স্থানে চুলকানি বা ঘষা করা, যা সাধারণত ঘুমানোর সময় ঘটে, পরজীবী শরীরে প্রবেশে সহায়ক হয়। একবার শরীরে প্রবেশ করলে, পরজীবীগুলি দ্রুত বংশবৃদ্ধি করে এবং ছড়িয়ে পড়ে।

চাগাস রোগ একজন সংক্রামিত ব্যক্তির কাছাকাছি থাকলে ছড়ায় না, তবে এটি বিভিন্ন উপায়ে ছড়াতে পারে:

  • পরজীবী সংক্রমিত পোকামাকড়ের মলযুক্ত অস্বাস্থ্যকর খাবার খাওয়া।

  • সংক্রামিত মায়ের গর্ভে জন্মগ্রহণ।

  • সংক্রামিত রক্ত বা অঙ্গ প্রতিস্থাপন করা।

  • পরীক্ষাগারে পরজীবী সংক্রমণের সম্ভাবনা।

  • বন্য প্রাণীর সংক্রমণের সম্ভাবনা, যেমন র্যাকুন বা অপসাম।
