Symptoms
The thighbone (femur) is the strongest bone in the human body, and breaking it requires significant force, making it usually obvious when fractured. However, fractures in the shinbone (tibia) or the fibula, which runs alongside the shinbone, may not always be immediately noticeable.
Signs and symptoms of a broken leg include:
-
Intense pain, which may worsen with movement
-
Swelling
-
Tenderness in the affected area
-
Bruising
-
Visible deformity or shortening of the leg
-
Difficulty or inability to walk
In young children or toddlers, a broken leg may cause limping or refusal to walk, even if they cannot explain their pain.
When to See a Doctor
If you or your child show any symptoms of a broken leg, seek medical attention immediately. Delayed diagnosis and treatment can lead to complications, including improper healing.
Seek emergency medical care for any leg fracture caused by high-impact trauma, such as a car or motorcycle accident. Thighbone fractures are serious and can be life-threatening, requiring immediate medical intervention to prevent further damage and ensure safe transportation to a hospital.
Causes
A broken leg can result from:
-
Falls: A simple fall may cause fractures in the lower leg bones, though breaking the thighbone requires a much stronger impact.
-
Motor Vehicle Accidents: High-impact crashes can fracture all three leg bones, especially if the knees hit the dashboard or external force impacts the legs.
-
Sports Injuries: A broken leg can occur due to excessive force during contact sports, overstretching the leg, a fall, or a direct impact (such as being hit with a hockey stick or colliding with another player).
-
Child Abuse: In infants and young children, an unexplained broken leg may sometimes indicate child abuse, especially if the child is not yet walking.
-
Overuse: Stress fractures are small cracks in the weight-bearing bones, including the shinbone. These often result from repetitive impact, such as long-distance running. A bone weakened by osteoporosis is more prone to stress fractures, even with normal activity.

Diagnosis
During a physical examination, the healthcare provider will check for tenderness, swelling, deformity, or an open wound in the affected area.
To determine the exact location and severity of the fracture, an X-ray is usually performed. In some cases, a CT scan (computerized tomography) or MRI (magnetic resonance imaging) may be required for more detailed imaging—especially for suspected stress fractures, which are often not visible on regular X-rays.
Treatment
The treatment approach depends on the type and location of the fracture. Some stress fractures may heal with rest and immobilization, while other types may require surgery for proper healing.
Types of Fractures:
-
Open fracture – The bone pierces the skin, requiring immediate medical treatment to prevent infection.
-
Closed fracture – The bone is broken, but the surrounding skin remains intact.
-
Incomplete fracture – The bone has cracked but is not completely broken into two parts.
-
Complete fracture – The bone is completely broken into two or more pieces.
-
Displaced fracture – The broken bone fragments are misaligned and may need surgical realignment.
-
Greenstick fracture – A partial fracture that occurs when the bone bends and cracks but does not break completely (common in children due to softer, more flexible bones).
Setting the Leg (Realignment & Immobilization)
-
Initial treatment starts in an emergency room or urgent care clinic, where the injured leg is immobilized using a splint.
-
If the fracture is displaced, the doctor may need to realign the bone fragments before applying a splint—a process called reduction.
-
Once the swelling subsides, a cast may be applied for better immobilization.
Immobilization
-
To heal correctly, the broken bone must remain stable and immobilized. A cast or splint is used to restrict movement.
-
Crutches or a cane may be necessary to keep weight off the injured leg for at least 6 weeks.
Medications
Pain relief may include:
-
Over-the-counter painkillers like acetaminophen (Tylenol) or ibuprofen (Advil, Motrin).
-
Stronger prescription painkillers for severe pain.
Rehabilitation & Physical Therapy
After the cast or splint is removed, physical therapy helps:
-
Regain movement and flexibility in the leg.
-
Strengthen weakened muscles that may have deteriorated due to immobilization.
-
Recovery may take several months for severe fractures.
Surgery & Other Procedures
Some fractures may require surgery to implant plates, rods, or screws to hold the bone in position during healing. This is more common in:
In some cases, external fixation is used—a metal frame attached to the bone with surgical pins to keep the leg stable. This is removed after 6–8 weeks but carries a risk of infection.
Preparing for Your Appointment
What You Can Do:
-
Write down symptoms and how the injury occurred.
-
Note any previous medical conditions.
-
List all medications and supplements being taken.
-
Prepare questions for the doctor, such as:
-
What tests are needed?
-
What is the best treatment option?
-
Is surgery necessary?
-
What are the alternatives?
-
What restrictions should be followed?
-
Do I need to see a specialist?
-
What pain medication do you recommend?
What to Expect from the Doctor:
The doctor may ask:
-
How did the injury happen?
-
Are there any other painful areas?
-
Have you had previous fractures or osteoporosis?
-
Do you feel numbness or tingling?
-
Were you able to stand or walk after the injury?
-
What makes the pain better or worse?
-
In children, doctors may ask routine questions to rule out child abuse.

(Immobilization)
