Symptoms
Aplastic anemia may not show any symptoms. When symptoms do occur, they can include:
- Fatigue
- Shortness of breath
- Rapid or irregular heart rate
- Pale skin
- Frequent or prolonged infections
- Unexplained or easy bruising
- Nosebleeds and bleeding gums
- Prolonged bleeding from cuts
- Skin rash
- Dizziness
- Headache
- Fever
Aplastic anemia can either resolve on its own or develop into a chronic condition. It can be severe and even life-threatening.
Causes
Stem cells in the bone marrow are responsible for producing blood cells — red blood cells, white blood cells, and platelets. In aplastic anemia, these stem cells are damaged, leading to an empty or underpopulated bone marrow.
The most common cause of aplastic anemia is when the immune system attacks these stem cells. Other factors that can damage bone marrow and impact blood cell production include:
- Radiation and chemotherapy: These cancer treatments, while targeting cancer cells, can also harm healthy cells, including stem cells in the bone marrow. Aplastic anemia may be a temporary side effect of these therapies.
- Exposure to toxic chemicals: Chemicals like those used in pesticides and insecticides, as well as benzene (found in gasoline), have been linked to aplastic anemia. Avoiding repeated exposure to these chemicals may improve the condition.
- Certain medications: Some drugs, such as those used to treat rheumatoid arthritis or certain antibiotics, can contribute to aplastic anemia.
- Autoimmune disorders: An autoimmune disorder, in which the body attacks healthy cells, may involve damage to the stem cells in the bone marrow.
- Viral infections: Viruses that affect the bone marrow, such as hepatitis, Epstein-Barr, cytomegalovirus, parvovirus B19, and HIV, have been associated with aplastic anemia.
- Pregnancy: In some cases, the immune system may attack the bone marrow during pregnancy.
- Unknown causes: In many instances, the cause of aplastic anemia remains unidentified (idiopathic aplastic anemia).
Connections with Other Rare Disorders
Some people with aplastic anemia may also have paroxysmal nocturnal hemoglobinuria, a rare disorder that causes red blood cells to break down too early. This condition may result in aplastic anemia, or aplastic anemia may develop into paroxysmal nocturnal hemoglobinuria.
Fanconi’s anemia, a rare inherited condition, can lead to aplastic anemia. Children born with this disorder may be smaller than average and have birth defects like underdeveloped limbs. Diagnosis is confirmed through blood tests.

Diagnosis
Several tests can help diagnose aplastic anemia:
-
Blood tests: Typically, red blood cells, white blood cells, and platelets are maintained within specific ranges. In aplastic anemia, all three blood cell types are lower than usual.
-
Bone marrow biopsy: A doctor removes a small sample of bone marrow from a large bone in the body, such as the hip, using a needle. This sample is then examined under a microscope to eliminate other blood-related diseases. In aplastic anemia, the bone marrow contains fewer blood cells than normal. A bone marrow biopsy is required to confirm the diagnosis.
Once the diagnosis of aplastic anemia is made, additional tests may be necessary to identify the cause.
Treatment
Treatment options for aplastic anemia depend on the severity of the condition and the patient’s age. These might include monitoring, blood transfusions, medications, or bone marrow transplantation. Severe aplastic anemia, in which blood cell counts are extremely low, is life-threatening and requires immediate hospitalization.
Blood Transfusions
While blood transfusions don’t cure aplastic anemia, they can manage bleeding and ease symptoms by supplying blood cells that the bone marrow isn’t producing. You may receive:
- Red blood cells: These help increase red blood cell counts and alleviate anemia and fatigue.
- Platelets: These help prevent excessive bleeding.
Although there’s generally no limit to how many blood transfusions you can receive, complications may arise with repeated transfusions. Transfused red blood cells contain iron that can accumulate in the body, potentially harming vital organs if not treated. Medication can help remove excess iron.
Over time, the body may develop antibodies to transfused blood cells, making them less effective. The use of immunosuppressant medications reduces the chances of this complication.
Stem Cell Transplant
For individuals with severe aplastic anemia, a stem cell transplant, also known as a bone marrow transplant, may be the most effective treatment. It is typically recommended for younger individuals with a matching donor, usually a sibling.
If a donor is found, the diseased bone marrow is first depleted through radiation or chemotherapy. Healthy stem cells from the donor are filtered from the blood and injected intravenously into the bloodstream. These healthy stem cells travel to the bone marrow, where they begin producing new blood cells.
This procedure requires an extended hospital stay. After the transplant, medications are prescribed to prevent the body from rejecting the donated stem cells.
A stem cell transplant carries risks, including the possibility of the body rejecting the transplant, which could lead to life-threatening complications. Not everyone is suitable for this procedure or can find a matching donor.
Immunosuppressants
For those who cannot undergo a bone marrow transplant, or if aplastic anemia is caused by an autoimmune disorder, treatment may involve medications that suppress the immune system. These include drugs like cyclosporine and anti-thymocyte globulin, which suppress immune cell activity and allow the bone marrow to recover and produce new blood cells.
These medications are often used together with corticosteroids such as methylprednisolone.
While effective, these drugs weaken the immune system, and there’s a possibility of anemia returning after discontinuing treatment.
Bone Marrow Stimulants
Certain drugs, including colony-stimulating factors like sargramostim, filgrastim, and pegfilgrastim, along with epoetin alfa and eltrombopag, help stimulate the bone marrow to produce new blood cells. These growth factors are often used alongside immunosuppressant medications.
Antibiotics and Antivirals
Aplastic anemia weakens the immune system, making you more susceptible to infections. If you develop a fever or any signs of infection, it’s crucial to see a doctor immediately. For severe cases, antibiotics or antivirals may be prescribed to prevent infections.
Other Treatments
If aplastic anemia is caused by cancer treatments such as radiation or chemotherapy, the condition typically improves once these treatments end. The same applies to most drugs that induce aplastic anemia.
Pregnant women with aplastic anemia typically receive blood transfusions. For many, pregnancy-related aplastic anemia improves after the pregnancy ends. However, treatment may still be required if the condition persists.
Self-Care
For individuals with aplastic anemia, taking care of your health is vital:
- Rest when necessary, as anemia can cause fatigue and shortness of breath with mild exertion.
- Avoid contact sports due to the risk of bleeding from a low platelet count.
- Protect yourself from germs by frequently washing your hands and avoiding sick individuals. Seek medical attention if you experience a fever or signs of infection.
Coping and Support
Managing aplastic anemia requires emotional and mental support. Some ways to cope with the condition include:
- Research your disease: The more knowledge you gain, the better you’ll be at making informed treatment decisions.
- Ask questions: Be proactive in discussing concerns and treatment options with your doctor.
- Express concerns: Don’t hesitate to voice any worries to your doctor or healthcare provider.
- Seek emotional support: Reach out to family and friends, and consider joining support groups. Your doctor may know of local groups or you can contact organizations such as the Aplastic Anemia and MDS International Foundation.
Preparing for Your Appointment
Before visiting the doctor, gather information to discuss, including your symptoms, medical history, and medications.
Questions to ask your doctor:
- What’s the most likely cause of my symptoms?
- Are there other possible causes for my symptoms?
- What treatments are available, and which do you recommend?
- How can I manage my condition with another health issue?
