Symptoms of Aortic Valve Disease
Some people with aortic valve disease may not experience symptoms for many years. However, when symptoms do occur, they may include:
- Chest pain or tightness
- Dizziness
- Fainting
- Fatigue after physical activity or reduced ability to remain active
- Irregular heartbeat
- Shortness of breath, especially during vigorous activity or when lying down
- Poor appetite, commonly seen in children with aortic valve stenosis

Causes of Aortic Valve Disease
Aortic valve disease may be caused by a heart issue present from birth, known as a congenital heart defect. Other causes of aortic valve disease that may develop later in life include:
- Age-related changes to the heart
- Infections
- High blood pressure
- Injury to the heart
To understand the causes of aortic valve disease, it’s helpful to first know how heart valves typically function. The heart contains four valves that ensure blood flows in the right direction:
- Aortic valve
- Mitral valve
- Tricuspid valve
- Pulmonary valve
Each valve has flaps, also known as cusps or leaflets, that open and close once during each heartbeat.
In aortic valve disease, the valve between the heart’s lower left chamber and the main artery (the aorta) doesn’t function properly. The left ventricle is the lower left heart chamber, and the aorta is the body’s main artery. The valve may become thick, stiff, or fail to close properly.

Diagnosis of Aortic Valve Disease
To diagnose aortic valve disease, a healthcare professional will examine you and ask about your symptoms and medical history.
A whooshing sound, called a heart murmur, may be heard when listening to the heart with a stethoscope. If this is detected, you may need to consult a cardiologist (a doctor trained in heart diseases).
Tests to Diagnose Aortic Valve Disease:
- Echocardiogram: This test uses sound waves to create images of the heart as it beats. It shows how blood flows through the heart and valves and helps assess the severity of the disease. There are different types of echocardiograms. A standard one is done from outside the body, with an ultrasound device pressed gently against the chest. If more details are needed, a transesophageal echocardiogram may be done, where the device is passed down the throat into the esophagus.
- Electrocardiogram (ECG or EKG): This test records the electrical activity of the heart and shows how it beats. Sticky patches are placed on the chest and sometimes on the legs, and wires connect them to a computer to display or print results.
- Chest X-ray: This shows the condition of the heart and lungs, helping to determine if the heart is enlarged, which could indicate aortic valve disease or heart failure.
- Cardiac MRI: This test uses magnetic fields and radio waves to create detailed images of the heart, helping to measure the severity of the disease.
- Cardiac CT Scan: This scan uses X-rays to produce detailed images of the heart and valves, useful for measuring the size of the aorta or assessing aortic valve stenosis.
- Exercise or Stress Tests: These tests monitor the heart’s response to physical activity, often performed while walking on a treadmill or cycling. If exercise is not possible, medication can be used to simulate physical activity.
- Cardiac Catheterization: Though not often used, this test involves inserting a tube into a blood vessel, usually in the groin or arm, to assess the severity of the disease or gather more detailed information about blood flow.
Staging of Aortic Valve Disease:
Once a diagnosis is confirmed, the disease is staged to determine the most appropriate treatment. The stages are:
- Stage A: At risk for heart valve disease
- Stage B: Mild or moderate valve disease without symptoms
- Stage C: Severe valve disease without symptoms
- Stage D: Severe valve disease with symptoms
Treatment for Aortic Valve Disease
Treatment depends on the severity, whether symptoms are present, and if the disease is progressing. Treatment options may include regular checkups, lifestyle changes, medications, or surgery. A multidisciplinary team, including cardiologists, may be involved in the treatment.
Medications:
If the disease is mild or moderate, regular checkups may be enough. For symptoms or complications, medications may be prescribed to:
- Lower blood pressure
- Prevent irregular heartbeats
- Remove excess fluid to reduce the heart’s strain
Surgery or Procedures:
If necessary, surgery or a catheter procedure can be used to repair or replace the damaged aortic valve. Surgery is often performed via open-heart surgery, though less invasive methods may be used.
- Aortic Valve Repair: Involves separating fused valve flaps, adding support, reshaping tissue, or patching holes.
- Aortic Valve Replacement: The damaged valve is replaced with a mechanical valve or one made from animal or human tissue. A tissue valve is called a biological tissue valve.
- Minimally Invasive Procedures: Some surgeries can be done with smaller incisions, such as Transcatheter Aortic Valve Replacement (TAVR).
Balloon Valvuloplasty:
