Albinism

Albinism (অ্যালবিনিজম )generally refers to oculocutaneous albinism (OCA), a group of inherited conditions where the body produces little or no melanin. Melanin is responsible for the color of the skin, hair, and eyes. It also plays a crucial role in eye development and function, which is why individuals with albinism often experience vision problems.

Signs of albinism typically appear in a person’s skin, hair, and eye color, though in some cases, these differences may be subtle. People with albinism are highly sensitive to sunlight, increasing their risk of skin cancer.

Although there is no cure for albinism, individuals can take protective measures to safeguard their skin and eyes while ensuring proper medical care for their vision and skin health.

যদিও অ্যালবিনিজমের কোনো নিরাময় নেই, তবে আক্রান্ত ব্যক্তিরা সঠিক যত্ন নিয়ে ত্বক ও চোখের সুরক্ষা নিশ্চিত করতে পারেন এবং প্রয়োজনীয় চিকিৎসা গ্রহণ করে স্বাস্থ্যের উন্নতি করতে পারেন।

Symptoms of Albinism

Albinism affects skin, hair, eye color, and vision.

Skin

The most noticeable form of albinism results in white hair and very light skin compared to family members. However, pigmentation in the skin and hair can range from white to brown. In people of African descent, albinism may present as light brown or reddish-brown skin, sometimes with freckles. Some individuals may have skin tones similar to their parents or siblings without albinism.

Sun exposure may cause:

  • Freckles.
  • Moles, which may be pink or without pigment.
  • Large, freckle-like spots called solar lentigines.
  • Sunburn and an inability to tan.

For some, skin color remains unchanged throughout life, while for others, melanin production may begin or increase during childhood or adolescence, leading to slight changes in pigmentation.

Hair

Hair color varies from white to brown. In people of African or Asian descent, hair may appear yellow, red, or brown. Hair color can darken during early adulthood or due to environmental factors like minerals in water, making hair appear darker over time.

Eye Color

Eyelashes and eyebrows are often pale. Eye color ranges from very light blue to brown and may change with age.

Due to a lack of sufficient pigment in the iris, light easily passes through, making the eyes highly sensitive to bright light. This can cause very light-colored eyes to appear red under certain lighting conditions.

Vision Problems

Vision impairment is a major characteristic of albinism and may include:

  • Nystagmus – Uncontrollable rapid eye movements.
  • Abnormal head posture – Tilting the head to compensate for vision issues.
  • Strabismus – Eyes not aligning or appearing crossed.
  • Farsightedness or nearsightedness – Difficulty seeing near or far objects.
  • Photophobia – Extreme sensitivity to light.
  • Astigmatism – Blurred vision due to irregular curvature of the eye’s surface.
  • Retinal underdevelopment – Reduced vision due to improper development of the retina.
  • Optic nerve misrouting – Nerve signals from the eye do not follow normal pathways to the brain.
  • Poor depth perception – Difficulty perceiving three-dimensional space.
  • Legal blindness (vision below 20/200) or, in rare cases, complete blindness.

When to See a Doctor

At birth, healthcare providers may notice lighter hair and skin, including pale eyelashes and eyebrows. They may recommend an eye examination and monitor any changes in vision and skin color.

If you notice signs of albinism in your child, consult a healthcare provider. Also, seek medical advice if a child with albinism frequently experiences nosebleeds, easy bruising, or recurrent infections, as these could indicate other rare hereditary conditions.

Causes of Albinism

Albinism occurs due to genetic changes affecting melanin production. Melanin is produced by melanocytes, which are found in the skin, hair, and eyes. Mutations in specific genes result in little to no melanin production, leading to different types of albinism.

Types of Albinism

Albinism is classified based on inheritance patterns and the affected genes.

  • Oculocutaneous Albinism (OCA) – The most common type, inherited when both parents pass down a mutated gene (autosomal recessive inheritance). OCA is caused by mutations in one of eight genes (OCA1 to OCA8) and affects skin, hair, and eye pigmentation, along with vision problems. Pigmentation levels vary depending on the specific gene affected.
  • Ocular Albinism – Primarily affects the eyes, causing vision issues. The most common form, type 1, is inherited through the X chromosome (X-linked recessive inheritance). Since males inherit only one X chromosome, they are more commonly affected. This type is much rarer than OCA.
  • Albinism Associated with Genetic Syndromes – Some rare conditions involve albinism along with other medical problems:
    • Hermansky-Pudlak Syndrome (HPS) – Includes OCA along with issues such as bleeding disorders, lung disease, and digestive problems.
    • Chediak-Higashi Syndrome (CHS) – Involves OCA, immune system dysfunction, recurrent infections, neurological issues, and bleeding disorders.

অ্যালবিনিজমের প্রভাব ত্বক, চুল, চোখের রঙ এবং দৃষ্টিশক্তিতে দেখা যায়।

ত্বক

অ্যালবিনিজমের সবচেয়ে সাধারণ লক্ষণ হলো সাদা চুল এবং তুলনামূলকভাবে ফর্সা ত্বক। তবে ত্বক ও চুলের রঙ সাদা থেকে বাদামি পর্যন্ত পরিবর্তিত হতে পারে। আফ্রিকান বংশোদ্ভূতদের মধ্যে হালকা বাদামি বা লালচে-বাদামি ত্বক এবং ফ্রিকেল দেখা যেতে পারে। কিছু ক্ষেত্রে, আক্রান্ত ব্যক্তির ত্বকের রঙ তার বাবা-মা বা ভাই-বোনদের মতোই হতে পারে।

সূর্যের সংস্পর্শে কিছু মানুষের ক্ষেত্রে দেখা দিতে পারে:

  • ফ্রিকেল।
  • গোলাপি বা রঙহীন আঁচিল।
  • বড় আকারের ফ্রিকেলের মতো দাগ, যা সোলার লেন্টিজিনেস নামে পরিচিত।
  • সহজেই রোদে পোড়া ও ত্বক কালো না হওয়া।

কিছু মানুষের ত্বকের রঙ সারাজীবন অপরিবর্তিত থাকে, আবার কিছু ক্ষেত্রে শিশুকাল বা কিশোর বয়সে মেলানিন উৎপাদন শুরু হতে পারে, ফলে হালকা পরিবর্তন দেখা যেতে পারে।

চুল

চুলের রঙ একেবারে সাদা থেকে বাদামি পর্যন্ত হতে পারে। আফ্রিকান বা এশিয়ান বংশোদ্ভূত ব্যক্তিদের ক্ষেত্রে চুলের রঙ হলুদ, লাল বা বাদামি হতে পারে। প্রাপ্তবয়স্ক হওয়ার সাথে সাথে চুল গাঢ় হতে পারে বা পরিবেশগত কারণে চুলের রঙ বদলাতে পারে।

চোখের রঙ

চোখের পাতা ও ভ্রু সাধারণত ফ্যাকাশে হয়। চোখের রঙ হালকা নীল থেকে বাদামি পর্যন্ত হতে পারে এবং বয়সের সাথে পরিবর্তিত হতে পারে।

চোখের আইরিসে পর্যাপ্ত মেলানিন না থাকায় আলো সহজেই প্রবেশ করে, যার ফলে উজ্জ্বল আলোতে চোখ অত্যন্ত সংবেদনশীল হয়। কখনো কখনো হালকা রঙের চোখ লালচে দেখাতে পারে।

দৃষ্টিজনিত সমস্যা

অ্যালবিনিজমে সাধারণত নিম্নলিখিত দৃষ্টির সমস্যাগুলো দেখা যায়:

  • নাইস্ট্যাগমাস – চোখের অনিয়ন্ত্রিত দ্রুত নড়াচড়া।
  • অস্বাভাবিক মাথার অবস্থান – দৃষ্টিশক্তির ভারসাম্য রক্ষায় মাথা কাত করে রাখা।
  • স্ট্র্যাবিসমাস – চোখের সামঞ্জস্যহীনতা বা কানা দেখা যাওয়া।
  • দূরদৃষ্টি বা নিকটদৃষ্টি – দূরের বা কাছের জিনিস স্পষ্টভাবে দেখতে সমস্যা।
  • ফটোফোবিয়া – তীব্র আলোক সংবেদনশীলতা।
  • অস্থিগম্যাটিজম – চোখের সামনের অংশের অস্বাভাবিক গঠনজনিত কারণে ঝাপসা দেখা।
  • রেটিনার অসম্পূর্ণ গঠন – যার ফলে দৃষ্টিশক্তি কমে যায়।
  • অপটিক নার্ভের ত্রুটি – চোখ থেকে মস্তিষ্কে সিগন্যাল যাওয়ার অস্বাভাবিকতা।
  • গভীরতা অনুধাবনের সমস্যা – ত্রিমাত্রিক বস্তু বুঝতে না পারা।
  • আইনি অন্ধত্ব (২০/২০০ এর কম দৃষ্টি) বা সম্পূর্ণ অন্ধত্ব।

চিকিৎসকের পরামর্শ নেওয়ার সময়

যদি শিশুর অ্যালবিনিজমের লক্ষণ দেখা যায়, তবে দ্রুত চিকিৎসকের পরামর্শ নেওয়া উচিত। অতিরিক্ত রক্তপাত, সহজে ক্ষত হওয়া বা দীর্ঘস্থায়ী সংক্রমণের মতো লক্ষণ দেখা দিলে তা হতে পারে বিরল বংশগত রোগের ইঙ্গিত।
